    REPEAT VIOLATION: Facility shall install a 1-compartment sink where drain line is lead to floor sink. [CA] A person proposing to build or remodel a food facility shall submit plans and specifications for review and shall receive plan approval prior to starting any new construction or remodeling. Contact DEH at 408-918-3400 or at www.ehinfo.org to obtain plan check requirements. Facility is hereby given 30 day from todays inspection to submit plans to Santa Clara County Health Department.

[CA] Install mounted paper towel dispenser and store single use paper towel in dispenser.

    Found bag of onions stored on floor near back prep area. [CA] Food shall be stored a minimum of 6 inches off the floor.

    Measured a tray of pork (lechon with fillings) sitting on the prep table for less than 4 hours holding at 118F. [CA] Maintain all hot PHFs held at or above 135F. [COS] Operator directed to reheat PHFs to 165F.

    Found hot water valve off at the front hand wash station. Observed leak at water valve when water valve is turned on. [CA] Repair leak at valve and ensure cold/hot water valve is provided at all times. Note: Observed an extension cord near water valve where leak occurs when water valve is turned on. Relocate extension cord to prevent from accidental electrocution.

    Found single use containers being reused for the storage of other food items. [CA] Food containers intended for single use shall not be reused. Provide approved multi use containers.
