FOLLOW-UP INSPECTION

This is a second follow-up inspection to routine inspection that was conducted on 05/16/25. The store was restricted from selling any food products due to vermin infestation. A pest control company came over yesterday and conducted intervention and advised the manager to seal all the pipe extensions. The store also has cleaned the store, seal the holes, caulk cracks and crevices, and stored food products to prevent vermin hiding and breading. No rodent droppings, no live or dead cockroaches, or traces of vermin was observed during the time of this inspection. Therefore, the store is now open and allowed to sell food products. Please conduct regular cleaning, address structural deficiencies, and hire a pest control company to conduct interventions. Please complete the rodent intervention form and email me immediately. Since this is a second follow-up for the same violation, the owner of the facility shall be billed $298.00 re-inspection fee.

FOLLOW-UP INSPECTION

Note: This is a follow-up inspection to the routine inspection that was conducted on 5/16/2025. The store was closed due to vermin infestation. The store has hired a pest control company which cleaned the dead and live cockroaches which were observed on and below the table where soda and other beverages dispensers are stored. However, the holes located above and behind the counters are still not sealed. In addition to that, couple of rodent droppings were on on the floor below the nuts storage aisles. A live cockroach was also observed on below the orange juice dispenser. Therefore, the store shall remain closed. A pest control company has to comeback and conduct intervention. The structural deficiencies located behind the soda dispenser and on the storage room shall be sealed before contacting our office. Please contact the district inspector for follow-up inspection. Since this is a free follow-up inspection. However, any subsequent follow-up inspection after first follow-up shall be billed $298/hour, minimum one hour, during normal business hours (Monday - Friday, 7:30 AM to 4:30 PM), and $645 for the first 2 hours, thereafter $298 each additional hour, during non-business hours, and upon inspector availability.

The owner of the facility must provide a pest control company report which indicate what interventions were conducted and observed violations and recommendations. The structural issues including the cracks, crevices, and gaps the wall behind the soda dispenser must be caulk or sealed. To prevent vermin hiding and breading and easy cleaning of the floor area, all food products must remove from the floor and stored on shelves or dunnage racks. Any operational and structural issues that contribute to the vermin infestation must also be addressed.

Note:1. The facility is closed due to live and multiple dead cockroaches and fresh rodent droppings on the counter and below the soda dispenser. The posted placard is property of the Santa Clara County Department of Environmental Health and shall not be removed, covered-up, relocated, tampered, or copied. Failure to comply may result in enforcement actions per County Ordinance Code Section B11-55. A facility found to be in continued operation without a permit may incur a penalty of three (3) times the operating permit fee, and fee(s) for re-inspection(s) charged at the current hourly rate approved by the Santa Clara County Board of Supervisors.
