OWNERSHIP CHANGE INFORMATION NEW FACILITY NAME: DIAMOND GAS & MARKET #9 NEW OWNER: RAJAB KHAN The applicant has completed the facility evaluation application process for an Environmental Health Permit. The permit category for this facility is FP06. An invoice for the permit fee in the amount of $1,165.50 will be mailed to the billing address on the application. Payment must be submitted within 10 days of receipt of the invoice. The owner is responsible for contacting our department if an invoice is not received and remit payment within 30 days. The Environmental Health Permit will be effective: 10/01/2024 - 09/30/2025 This report serves as a temporary permit. However, the permit will be deemed invalid if the permit fee is not paid in full within 30 days from the date of this report. Okay to Operate. An official permit will be mailed to the address on file and shall be posted in public view upon receipt. *Structural Review inspection conducted on 09/19/2024

Performed a routine inspection on 6/7/2024 and talked to a manager on phone and informed him about a change of ownership requirement. On 9/6/24 talked to Rajab Khan 510-501-6949 and give out final 48 hour notice. Facility is unable to complete ownership change and operating without permit. Facility must contact the department by Monday 9/9/2024 and submit CHANGE OF OWNERSHIP application or we will close facility's Food operation on 9/10/2024. *"Unable to complete ownership change.** Operation since January 2024 without permit issued.
