Spider webs and dirt accumulation was observed on the walls above the storage area . [CA] Conduct thorough regular cleaning on the walls. There is a hole in the ceiling in the meat department above one of the handwash stations. [CA] To prevent vermin access, please seal the hole on the ceiling.

Follow up for no hot water during limited inspection on 1/21/2026. Measured hot water at all sinks. Meat Dept. is approved to reopen. PIC provided via email spec sheets for new water heater. ROUTINE INSPECTION

Limited inspection conducted due to major violation of no hot water. Per PIC, the dept. has not had hot water since the day before and is schedule to have the water heater replaced in the next day or two. Due to no hot water, no active preparation or ware washing can be conducted. Grocery store is to partially close and must cease and desist any food handling/ preparation in the MEAT dept. Grocery store can continue to sell prepackaged food items. Once the above is corrected, please reach out to the Jennie Penaloza at 408-918-3471 or the main office at 408-918-3400 to schedule a follow-up inspection to reopen the facility. ***Subsequent follow up inspection after first follow up shall be billed $298/hr during business hours and $645/hrs (minimum of 2 hours) during nonbusiness hours upon availability. *** *** Measured hot water (123F - 138F) at the POLLO Dept. (PR0425839) and the TAQUERIA dept. (PR0376456). These departments can remain open however no food can be obtained from the meat dept. to supply the Pollo Dept. or Taqueria. Meat Dept is to remain closed. Discussed with PIC, that Plan Check review and approval will be required if new water heater is a different model/type from the current one. Reinspection for inspection conducted on 02/06/2025. Major violations were corrected. Hand wash stations were fully stocked. Paper towel dispenser is changed and dispense paper towel properly. No products observed left outside the walk-in fridge. 3 compartment sink plumbing is repaired. Discussed with PIC regarding relocating creameria to Deli department instead of meat department to prevent potential cross contamination. Continue to correct violations dated on 02/06/2025 report.

[SA] Caution tape is used around 3 compartment sink to alert employee stop using 3 compartment sink. Warewash will be conducted at deli area at the meantime.

Observed 1 piece of black foam is deteriorated from the foam. [CA] Surfaces of equipment such as cutting blocks and boards that are scratched and scored so that they can no longer be effectively be cleaned and sanitized shall be resurfaced or replaced. Heavy debris built up on the produce fogger. [CA] Produce fogger shall be cleaned and maintained in accordance with manufacturer's specifications.
