Roll up doors observed to have gaps underneath doors at lest 1/4" in diameter. [CA] Seal gaps to prevent vermin entry.
- K06: Adequate handwash facilities supplied, accessibleCritical
Handwash sink missing soap. No other handwash sink available in facility and being used by dippin dots stand staff during inspection. [CA] Keep all handwash sinks fully stocked with soap and paper towels at all times. [COS] Hand soap was obtained and dispenser was restocked
- K45: Floor, walls, ceilings: built,maintained, clean
Floors observed to be missing tiles and cove basing especially where partial wall was removed. Spiderwebs observed on floor and around facility. [CA] Clean floors, walls and ceilings. Maintain finishes in good repair.

Compressed air tank observed to be unsecured. [CA] Secure air tank to rigid structure
- K21: Hot and cold water available
Hot water measured to be 110F at 3-comp sink. [CA] Provide hot water at 120F at 3-compartment sink.
