The permit category for this facility is FP10. An invoice for the permit fee in the amount of $1,107.00 will be mailed to the billing address on the application. Payment must be submitted within 10 days of receipt of the invoice. The owner is responsible for contacting our department if an invoice is not received and remit payment within 30 days. FOLLOW-UP INSPECTION

Follow up inspection conducted for routine inspection on 12/09/2025 regarding rodent droppings. Specialist did not observe evidence of vermin after thorough inspection. Pest control service (12/10/2025) report from Western Exterminator Company provided and it states that the technician found holes/gaps on wall and between pipes. Upon inspection, noted holes/gaps have been sealed. Technician did not note any vermin activity in the kitchen area on the report. Continue to follow pest control service recommendations and monitor for vermin activity.
