To-go containers used as scoop for bulk ingredients. [CA] Use approved utensils such as scoops or spoons with handles to ensure hands do not come in direct contact with food.

Heavy accumulation of grime and/or food residues observed in the following areas: - Around the door handles of the island preparation refrigerator. - Outside and inside the two-door upright freezer, including door handles. Heavy accumulation of grease and food debris on the catch tray below the wok line. [CA] Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris.

Heavy build up of grime observed on floors: - Corner of the kitchen where the rice cookers are maintained. - In front of the janitorial sink and around the bulk ingredient bins. - Underneath the cook line. - Underneath storage rack and outside of the walk-in refrigerator. [CA] Walls and/or floors in food preparation area shall be kept clean. Regularly clean under equipment to prevent accumulation of debris.

Five-year Food Safety Certificate (FSC) is expired. [CA] Food facilities that prepare, handle, or serve non-prepackaged potentially hazard foods shall have a valid Food Safety Certificate available for review at all times. Employees' three-year food handler cards are expired. Only one valid food handler card was available. [CA] Each food handler shall maintain a valid food handler card for the duration of his or her employment as food handler. A valid food handler card shall be provided within 30 days of after the date of hire.

Paper towels maintained on top of the dispenser in the kitchen. [CA] Paper towels shall be stored inside the dispenser to prevent potential contamination. *** Facility does not have a dedicated hand wash sink. If hand washing violations are observed in future inspections and/or change of ownership, facility will be required to install dedicated hand wash sink.

Two large metal containers of cooked white rice maintained inside the walk-in refrigerator measured between 45F to 47F. Chef stated that the rice was from the night before. [CA] Potentially Hazardous Foods (PHFs) shall be cooled rapidly from 135°F to 70°F within 2 hours and from 70°F to 41°F within 4 hours. Approved cooling methods include: 1) Placing the food in shallow pans 2) Separating the food into thinner or smaller portions. 3) Using rapid cooling equipment (Ex. blast chiller) 4) Using containers that facilitate heat transfer (ex. stainless steel) 5) Adding ice as an ingredient. 6) Using ice paddles 7) Using an ice bath and stirring frequently 8) Accordance with a HACCP plan. Cooling foods shall have enough space around the containers for cold air to circulate, be loosely covered, or uncovered and stirred as frequently needed to evenly cool. [COS] Rice was VC&D due to improper cooling.
