One dead cockroach observed on sticky monitoring trap placed below the drain of the warewashing area. Per owner, facility was serviced same day. Pest control report was provided for review. No other activity was observed. [CA] Clean and sanitize area of dead cockroaches.

Flat of raw shelled maintained above ready-to-eat food items (pots of meat balls, steamed buns, etc.). [CA] All food shall be separated and protected from cross-contamination. Store all raw meat or other raw products of animal origin below ready-to-eat food items.

Observed 10+ rodent droppings in the following areas: - On floor along the wall underneath the tables and drain boards ware washing area. - On floor underneath the cook line. No observations of contaminated food nor food contact surfaces. Facility is serviced monthly with pest control scheduled for service today. [CA] Clean and sanitize area of old droppings.

Observed 2-3 bags of food stored directly on floor inside the walk-in freezer. [CA] Food shall be stored at least 6 inches above the floor to prevent contamination.

Observed large gap on wall where the sewage pipes protrude from the wall at the drain boards of the ware washing area - left of the sinks [CA] Repair holes in structure and maintain in good repair to prevent pest harborage.
