Cardboard and clutter observed to be stored around back door and in office area. [CA] Clean clutter and remove cardboard and maintain facility tidy and free of cluttter

[CA] Clean and sanitize area of dead cockroaches or old droppings.
1 singular prepped square pizza in zone 1 of pizza rack near oven at time of inspection. Timer observed to not be on. [CA] When time only, rather than time and temperature is used as a public health control, PHFs shall be time marked to indicate when item is removed from temperature control, discarded if not consumed or served within 4 hours, and written procedures shall be readily available for review. [COS] Per operator square prepped pizzas are only available for during lunch time between 11AM and 2:30PM and then square pizzas are discarded.
Domestic cooler observed to be used to store chicken wings. [CA] All utensils and equipment shall be certified or classified for sanitation by an American National Standards Institute (ANSI) accredited certification program (e.g. NSF, UL EPH, UL Sanitation, CSA Sanitation, or ETL Sanitation).
ROUTINE INSPECTION
Due to technical difficulties, this report will be emailed to the facility.
Observed accumulation of dust on fan covers inside the walk-in refrigerator. [CA] Keep fan covers clean.
1) Observed a large fan was placed in front of the kitchen handsink, partially obstructing it. [CA] Do not place items in front of the handsink. Handsink must be easily accessible at all times. 2) Observed a bar of soap was located next to the rear handsink (a wall-mounted liquid soap dispenser was also available). [CA] Discontinue using bar soap. Only liquid soap from a dispenser is approved.
Observed employee rinsed her hands in the 3-compartment sink. [CA] Hands must be washed at the handsink. [COS] Employee washed hands at the handsink.
