FOLLOW-UP INSPECTION

This is a follow up inspection to verify that all major violations mentioned on 11/10/25 inspection report have been addressed. Major violations have been addressed and found within compliance: - K23: No rodents, insects, birds, or animals. No evidence of vermin activity within the facility at the time of inspection. Facility has contracted pest control to come four times within the first month and then three times within the next month. Continue to monitor for any pest and notify pest control if issues arise. Provide reports of pest control to specialist for the next month. Continue on working on any other violations mentioned on 11/10/25

On site for owner requested follow up inspection. Specialist observed live cockroach activity during inspection. Facility shall remain closed until re-inspected by this department and approved to reopen. 1. Observed Vermin: Documented in the following areas: - 2 German cockroaches observed behind FRP behind far right beer refrigeration unit. - 1 live German cockroach observed behind electrical plug behind the far right beer refrigeration unit. - 1 live cockroach nymph observed in cabinet under the bar 3. Supervisor Notified: yes 4. Notification: The person in charge during inspection, Alicia Garcia, has been informed that the facility must remain close. [CA]: The premises of each food facility must be maintained free of vermin. A facility cannot operate if there is a vermin infestation that leads to contamination of food contact surfaces, packaging, utensils, food equipment, or adulteration of food. The facility is required to cease operations immediately and must remain closed until all corrective actions on the provided checklist are completed. Requirements Before Reopening: 1. Email the signed and completed Reopening Checklist to the assigned inspector. 2. Submit a copy of the pest control report from a licensed provider. Observed the taquero in the back removed. Owner does not plan on using it again. Subsequent follow-up inspection after first follow-up shall be billed $298/hour, minimum one hour, during normal business hours (Monday - Friday, 7:30 AM to 4:30 PM), and $645 for a minimum two hours, during non-business hours, and upon inspector availability.
