--48 HOUR NOTICE— Our records indicate this facility is operating without a valid permit and has a delinquent account. Your balance due is $ 1975.00. Payment must be made immediately to DEH at the address at the top of this report or via our website at www.Ehinfo.org. A facility that submits payment of the outstanding balance within 48 hours (2 business days) of this notice will be issued and mailed an operating permit by this department. Failure to comply within 48 hours will result in facility closure and additional enforcement, including but not limited to reinspection(s) and additional cost recovery fees. A facility found to be in continued operation without a permit may incur a penalty of three (3) times the operating permit fee, and fee(s) for re-inspection(s) charged at the current hourly rate approved by the Santa Clara County Board of Supervisors. Penalties and fees incurred due to enforcement activities must be paid in full prior to the issuance of an operating permit.

Several shelves have clutter and unorganized storage areas. [CA] Thoroughly clean and organize storage shelves. Observed personal items stored on shelves suck as supplements and peroxide. [CA] these items must be stored in lockers with all other personal items.

The green cutting boards are deteriorated with plastic particles falling off of them [CA] Discontinue use and repair with approved coting boards. Observed not working and unused refrigerators. [CA] Remove them from the facility.

The lighting is dim in several areas where food and/or drinks are stored or prepared. The lighting inside the walk in refrigerator is also dim. [CA] this lighting must be increased in these areas for proper cleaning and food safety.

The beer tap machine is draining into a bucket full of water. [CA] Proper plumbing and disposal is required of the waste water.

Observed the storage room floor to be very deteriorated. [CA] Repair the floor.

Observed drain pipes from food prep sinks and other items in the floor sink. [CA] Provide an air gap between the top of the floor sink and the drain pipe.
