- K45: Floor, walls, ceilings: built,maintained, clean
Excessive water accumulation was observed on the floor below front coffee brewing machine. [CA] Keep the floor areas neat, clean, and dry all the time.
- K23: No rodents, insects, birds, or animals
Couple of dry rat droppings were observed by the water heater. [CA] Eliminate all insects and rodents from the facility by a legal method. A couple of dead cockroaches were observed below the storage shelves located near the water heater. [CA] Eliminate vermin from the facility through proper storage of food, utensils and equipment, conducting regular cleaning, and performing a regular professional pest control intervention.

Repeated violation: Many food items in front self service cooler found without labels. [CA] Food prepackaged in a food facility shall bear a label with the following: 1) common name of the food, 2) list of ingredients in descending order of predominance by weight, 3) quantity of contents, 4) name and place of business of the manufacturer, packer, or distributor.

Many food products on boxes were stored directly on the floor or on milk crate back in the storage area. [CA] To prevent insect hiding and breading and for easy cleaning of the floor area, please store all food products at least six inches off the floor on an approved shelf or dunnage rack.

The front main door was left open. [CA] To prevent insect/rodent infestation, keep the doors always closed.
- K27: Food separated and protected
Raw food products like shell eggs were stored above ready to eat food products inside the walk-in cooler. [CA] To prevent possible contamination, ready-to-eat food products and produce must be stored above or by the said of raw food products.

Repeated violation: Potentially Hazardous Foods (PHFs) like sweet rice, chicken, shrimp rolls on display are subject to TPHC but owner forgot to put time stickers for today. These foods are prepared at 11 am and placed here for sale. [CA] When time only, rather than time and temperature is used as a public health control, PHFs shall be time marked to indicate when item is removed from temperature control, discarded if not consumed or served within 4 hours, and written procedures shall be readily available for review. [COS] Owner will discard the leftover food items after 4 hours of start time (around 3 pm) . Told owner to always post time stickers on each food.
