    3-4 live fruit/drain flies observed in the dough preparation room. - Owner stated that he suspects the flies were coming from the floor sink for the walk-in refrigerator and thus was sealed. 2-3 live fruit/drain flies observed in the ware washing area/janitorial sink. - Owner stated that the janitorial sink is not working and the drain should have been plugged, however, yesterday upon arrival after the closure, they had found that the drain was not covered. - Owner also stated that the janitorial sink was damaged and thus is not being used by the previous owner. As an alternative, the previous owner had installed a utility sink by the food preparation sink. 2-3 live fruit/drain flies observed in the dining area. Owner stated that the facility currently has two pest control companies to address the issue. - One of the pest control companies is scheduled to treat the facility today. - Last service was conducted on 10/14/2024. Pest control reports for service was unavailable for review. - All areas of the kitchen were cleaned and sanitized of opened food and food debris. - Since last closure, facility has increased frequency of treatment to 1-2 times per week. Employee's stated that hot sanitizing solution is poured down all floor sinks every hour and at night when facility is closed. ROUTINE INSPECTION

Note: 1. A customer reported he ate a pizza at this location with his kids and observed gnats everywhere in the dinning area. He also mentioned he got a stomach upset from eating the pizza. During the time of this inspection, many gnats were observed in the dining area, dough mixer, pizza preparation, and wash sink areas. Couple of flies were observed landing on the dough kneading table, dough mixing machine, salad bar, and on the wall where the hot pizza is stored. Since many flies were observed landing on equipment which results in possible contamination, the facility is closed until the issue comes under control. 2. Since this is a third follow-up to the fly complaint issue, the owner shall be called for office hearing. The owner shall meet with my supervisor.

The facility was closed due to drain fly infestation. During today's follow-up inspection, no dead or live flies were observed in the serving, preparation, or storage areas. Therefore, the facility is open for service. However, the above violations must be corrected immediately.
