ROUTINE INSPECTION
The facility is herby closed by this department with regards to vermin activity within the facility at the time of inspection. Subsequent follow-up inspections after the first follow-up shall be billed $298-hour, minimum one hour, during normal business hours: Monday-Friday 7:30AM to 5:00PM or $645/hour, minimum one hour, during non-business hours, and upon inspector availability. 1. Observed Vermin: Documented in the following areas: Observed a live field mice inside of a bucket under the 3-compartment sink at the time of inspection. 2. Photographs: Taken for documentation purposes. 3. Supervisor Notified: Elizabeth T. 4. Notification: The person in charge during inspection, Gino F., has been informed that the facility must close immediately. [CA]: The premises of each food facility must be maintained free of vermin. A facility cannot operate if there is a vermin infestation that leads to contamination of food contact surfaces, packaging, utensils, food equipment, or adulteration of food. The facility is required to cease operations immediately and must remain closed until all corrective actions on the provided checklist are completed. Requirements Before Reopening: 1. Email the signed and completed Reopening Checklist to the assigned inspector. 2. Submit a copy of the pest control report from a licensed provider.

Dish washer was measured at 0PPM at the time of inspection. No active ware-washing. [CA] Maintain chlorine in mechanical ware-wash machine at 50 PPM. [SA] Facility has a 3-compartment sink that will be used to sanitize equipment.
ROUTINE INSPECTION
Sanitizer within the dish washer was measured at 200PPM (Chlorine) at the time of inspection. No active ware-washing. [CA] Maintain chlorine in mechanical warewash machine at 50 PPM.
Two light bulbs are missing within the hood system at the time of inspection. [CA] Replace the light bulbs.
