- K39: Thermometers provided, accurate
There was not a functional prob thermometer to measure the internal temperature of the cold and hot food products. [CA] Provide an approved prob type thermometer and use it with regular calibration and sanitization.
- K27: Food separated and protected
Some open bag food products were stored on the shelf of in the dry storage and walk-in cooler areas. [CA] Store all open bag food products in properly labeled bulk containers.
- K32: Food properly labeled and honestly presented
Some individually packaged juice, dessert, snacks, and others made in the facility but stored on the self-serve area do not have any label. [CA] Food prepackaged in a food facility shall bear a label that complies with the labeling requirements prescribed by the Sherman Food, Drug, and Cosmetic Law, Part 5 and label information shall include: 1. The common name of the food 2. If made from two or more ingredients, a list of ingredients in descending order of predominance by weight, including a declaration of artificial color or flavor and chemical preservatives, if contained in the food. 3. An accurate declaration of the quantity of contents. 4. The name and place of business of the manufacturer, packer, or distributor.

Employees working in the back in the kitchen area was observed eating while preparing food. [CA] Employees may only drink or eat in designated areas. If employees must drink in the food preparation area, they must use covered cup container with a straw. Their drinks must also be stored in designated areas to prevent possible contamination.

Many food products on boxes were stored directly on the floor or on milk crate back in the storage and walk-in cooler/freezer areas. [CA] To prevent insect hiding and breading and for easy cleaning of the floor area, please store all food products at least six inches off the floor on an approved shelf or dunnage rack.

There is not a food safety manager certified person in the facility. [CA] Someone from the facility must take a food safety certificate class within two months and keep a copy of the certificate on file. The maximum water temperature on the mechanical dishwasher was measured at 151 OF. [CA] If high temperature mechanical dishwasher is used to sanitize dishes, the minimum temperature must be adjusted to 160 OF. Until the mechanical dishwasher's temperature is raised to 160 OF, sanitizing must be done on the two-compartment wash sink (SA). Some slim buildup was observed inside the ice machine’s internal part. [CA] Clean the internal part of the ice machine immediately.

Shell eggs stored on the preparation table were measured at 68 oF [CA] All potentially hazardous foods like meat, seafood, poultry, dairy products, cooked vegetables, cooked cereals, greens, and diced tomatoes shall always be held at 41°F or below or at 135°F or above. The chef said the eggs were taken out of the walk-in cooler two hours ago. Therefore, about four dozen shell eggs were voluntarily condemned and destroyed by the owner (COS).

Many drain flies were observed back in the janitorial sink area. [CA] Eliminate vermin from the facility by a legal method.
- K40: Wiping cloths: properly used, stored
Multiple wet/soiled wiping cloths were stored on the counters, preparation tables, and sinks. [CA] Wet/soiled wiping cloths must be stored in a bucket that has standard sanitizer concentration in between use which is 200 ppm quaternary ammonia or 100 ppm chlorine (bleach).
