FOLLOW-UP INSPECTION
This was a follow-up to the 8/5/25 routine inspection to assess compliance for presence of cockroaches and lack of paper towels. Observed paper towel dispensers in the rear area and restroom were stocked. Due to lack of major violations, the yellow placard was replaced with a green placard. Note: per PIC, the current owner name on file GIGI'S ORIENTAL MART INC may not be accurate. PIC will confirm. Recommend coming to the DEH office as soon as possible to discuss and potentially apply for a new permit under the new owner. Be advised, if the current owner does not have a valid environmental health permit, the new owner must come to our office and apply for and pay for a change of ownership application within 48 hours. Failure to comply may result in enforcement action, including facility closure for operating without a permit. Due to printer issues, this report was emailed to the facility during the inspection.

Observed 3 deceased cockroaches on the floor in the warewashing area, next to the chest freezer. No other evidence of vermin was observed. [CA] Keep facility clear of cockroaches.
