The permit category for this facility is FP17. An invoice for the permit fee in the amount of $2,011.00 will be mailed to the billing address on the application. Payment must be submitted within 10 days of receipt of the invoice. The owner is responsible for contacting our department if an invoice is not received and remit payment within 30 days. The Environmental Health Permit will be effective: 10/01/25 - 09/30/26 This report serves as a temporary permit. However, the permit will be deemed invalid if the permit fee is not paid in full within 30 days from the date of this report. Okay to Operate. FOLLOW-UP INSPECTION

This is a follow up inspection to verify compliance with major violation K23. Observations: K14: The high temp dishwasher was set up to run as a low temp dishwasher until they receive a missing part. Chlorine measured at 50 ppm. K23: Facility was serviced by EcoLab on 3/13/25 in which a fogging was provided. Observed a couple of dead adult cockroaches. Also observed several oothecas. Staff disposed of the oothecas immediately. Discussed the Bakery/Butcher program record needing to be reactivated. Provided FEA application. Submit within 48 hours. Continue to correct any and all violations.

    In the ware washing room, separated by two doors and a landing with the elevators from the main kitchen, numerous live roaches were observed. Live roaches were observed in the dishwasher and the wall nearest to the dishwasher. Adults and nymphs were both dead and alive were found. Per EcoLab service report, the facility was treated on 2/24/25 with a crack and crevice treatment. [CA] The premises of each food facility shall be kept free of vermin. A food facility shall not operate when there is a vermin infestation that has resulted in the contamination of food contact surfaces, food packaging, utensils, food equipment, or adulteration of food(s). The food facility shall cease operation of the food facility immediately. The food facility shall remain closed until: there is no longer evidence of a vermin infestation; all contaminated surfaces have been cleaned and sanitized; and contributing factors such as cleaning, repairs, and the elimination of harborages have been resolved. [SA] Facility will wash all dishes for the main kitchen in the Fountain Restaurant until cockroaches are abated. A follow up inspection will be conducted on 3/14/25.

    1. In the bakery area, there is a leak coming from the lights of the exhaust hood. 2. In the ware washing area there are several areas of the wall and floor that are in disrepair. [CA] Ensure to maintain the floors, walls, and ceilings of the facility clean and in good repair.

    The high temperature dishwasher reached a maximum temperature of 154F. No active warewashing took place. [CA] Mechanical sanitization shall be accomplished in the final sanitizing rinse by achieving a utensil surface temperature of 160°F (or manufacturer's instruction).
