    Observed hard boiled eggs being cooled in the food prep cold hold unit (the hardboiled eggs measured between 52F-54F). Per employee, the eggs were boiled at 11am. [CA] Ensure PHFs are properly cooled from 135F to 70F wtihin 2 hours and from 70F to 41F within 4 hours. Discontinue cooling PHFs in the food prep unit; must be done in the walk-in cooler or reach-in cooler. [COS] PIC relocated the hardboiled eggs to the walk-in cooler.

FOLLOW-UP INSPECTION

A follow-up inspection was conducted to verify major violation from routine inspection and follow-up inspection conducted on 06/27/2025, 07/01/2025, 07/02/2025 is in compliance. K23: No live or dead cockroaches observed in the facility. Pest control report dated 06/30/2025 shows pest activity and was treated. Per operator, the facility was fumigated on 07/02/2025. Pest control report dated 07/03/2025 indicate that technician did not observed any live or dead cockroaches. Facility OK to re-open after all food contact surfaces are cleaned and wiped down with sanitizer solution of 100 ppm of chlorine or 200 ppm of quaternary ammonium. Ensure ice is discarded as well. Continue to maintain facility free of pest and vermin. Discussed with operator to drain sushi boat water every night and maintain dry to prevent drinking well for cockroaches. Continue to work on minor violations on last routine inspection report. This is a owner initiated re-inspection after hours/holiday and will be charge $645.00. Failure to remit payment within 48 hours after receiving the invoice will result in a closure until paid.

    Observed 7 live cockroaches in different life stages and one ootheca (cockroach egg sack) as well as about 10 dead cockroaches. Cockroaches were found under food pass trhough window, shelving brackets, floor drains, by cookline , under equipment, and on food prep surface. [CA] Work with professional pest control and recommend setting a minimum of 20 additional monitoring traps. Ensure cockroaches and feces has been completely eliminated from the facility. Facility not ok to reopen.

Live and dead German cockroaches in some areas: - in sushi prep area - under and above wall mounted shelvings in kitchen - under soda machine - inside hand soap dispensers - in ceiling light fixtures - under dish machine - on food cutting boards - throughout kitchen floors - inside the water heater - near back door - by tea dispenser Observed severe cockroach droppings on walls under wall mounted shelves. ---B. Observed rodent droppings in the following locations within kitchen: - on wall corner next to sushi boat water ring - on wall and shelf above 3 comp sink - on top of 2 door reach in 2.     No sanitizer bucket observed at sushi prep station; employee is using a wet wiping cloth only with no sanitizer; no active cleaning observed.     Cooked fried shrimp is stored on the counter top in the sushi prep area; per staff, sushi was cooked 1 hour before inspection and is using time as a temperature control; no time stamp is provided for the shrimp. [CA] When time only, rather than time and temperature is used as a public health control, PHFs shall be time marked to indicate when item is removed from temperature control, discarded if not consumed or served within 4 hours, and written procedures shall be readily available for review.
