  • K38: Adequate ventilation/lighting; designated areas, use

    Gas cooker is not under ventilation. [CA] Provide approved exhaust hood ventilation over all cooking equipment as required to effectively remove cooking odors, smoke, steam, grease, heat, and vapors. Observed ovens and two burner stock pot under ventilation hood without 6" overhang on either side. [CA] There shall be a minimum of 6 inches overhang of ventilation hood over cooking equipment. Lights were not working in ventilation hood. [CA] Repair/replace lights. Observed heavy accumulation of grease on hoods in cooking area. [CA] Clean mechanical exhaust hood and/or filters of grease accumulation.

    Employees keys and purse are stored on shelving with food equipment. [CA] No person shall store clothing or personal effects in any area used for the storage and preparation of food or food contact surfaces.

    Observed panels installed with gaps or missing creating gaps in ceiling/walls above the walk in fridge. [CA] Repair holes in structure and maintain in good repair to prevent pest harborage. Observed heavy accumulation of food/debris on walls and floors throughout facility. [CA] Walls and/or floors in food preparation area shall be kept clean. Observed missing caulking in the coving throughout the facility. [CA] Repair/replace caulking in coving. Observed heavy accumulation of food grease/debris around cooking equipment. [CA] Walls and/or floors in food preparation area shall be kept clean.

    Observed pork and fish thawing in standing water at preparation sink. [CA] Frozen potentially hazardous food shall only be thawed in one of the following ways: 1) under refrigeration that maintains the food temperature at 41°F or below, 2) completely submerged under potable running water for a period not to exceed two hours at a water temperature of 70°F or below, and with sufficient water velocity to agitate and flush off loose particles into the sink drain, 3) in a microwave oven if immediately followed by immediate preparation, 4) as part of a cooking process.

    Observed raw chicken stored above vegetables in the walk in fridge. [CA] All food shall be separated and protected from cross-contamination. Store all raw meat or other raw products of animal origin below ready-to-eat food items.

    Observed bowls used as scoops in bulk food storage containers. [CA] Provide scoops with handles and store scoop handle in such a way that the handle does not come in direct contact with food to prevent contamination.

    Wheel broken on bulk food storage container. [CA] Repair/replace container and maintain in good condition. Observed heavily stained/scored/discolored cutting boards out for use. [CA] Surfaces of equipment such as cutting blocks and boards that are scratched and scored so that they can no longer be effectively be cleaned and sanitized shall be resurfaced or replaced.

    Clams in tanks did not have shellstock tags available. Receipts were provided for grocery store they were purchased from. [CA] The identity of the source of shellstock that are sold or served shall be maintained for 90 calendar days from the dates of harvest by using an approved recordkeeping system that keeps the tags or labels in chronological order correlated to the date or dates the shellstock are sold or served. [COS] Clams will be used for personal consumption.

    Observed one live cockroach on floor next to cookline. No other evidence of cockroaches observed. Reviewed [CA] Maintain facility free of disease causing vermin like cockroaches. [COS] PIC eliminated cockroach.

    Handwash sink was leaking underneath at drain. [CA] Repair plumbing fixture and maintain in clean and good repair.

  • K07: Proper hot and cold holding temperaturesCritical

    Noodles in cold holding insert measured 54F. Per PIC, it had been brought out of walk in fridge 40 minutes prior. Rice noodles on counter measured 78F, noodles had been delivered earlier in the morning. No preparation observed. [CA] PHFs shall be held at 41°F or below or at 135°F or above. [COS] Noodles were placed back into the reach in fridge.
