    Observed numerous prepackaged raw comminuted fish paste thawing in ambient temperature. [CA] Frozen potentially hazardous food shall only be thawed in one of the following ways: 1) under refrigeration that maintains the food temperature at 41°F or below, 2) completely submerged under potable running water for a period not to exceed two hours at a water temperature of 70°F or below, and with sufficient water velocity to agitate and flush off loose particles into the sink drain, 3) in a microwave oven if immediately followed by immediate preparation, 4) as part of a cooking process.

- On-site for a follow-up inspection after facility was closed due to evidence of a vermin infestation on a routine inspection on 11/10/2025. - Facility has corrected the following: - K23M - Observed rodents, insects, birds, or animals: - Facility has been serviced on pest control service, once before the routine inspection on 11/9/2025, again on 11/12/2025. - Facility has cleaned and sanitized the affected areas. - At time of inspection, no live or dead activity was observed. - Facility is hereby authorized to reopen and resume operations. - Continue to address all other violations noted on previous inspection reports. - Prior to reopening, conduct thorough cleaning and sanitization of all food contact surfaces.

ROUTINE INSPECTION

- Facility is hereby closed due to evidence of a vermin infestation. - Facility is to immediately cease and desist all operations. - Facility is prohibited from all food service activities which include, but not limited to: preparation, service, handling, distribution, and sale. - Facility shall remain closed until vermin infestation is completely abated and authorization is provided by this Division. - Facility shall: - Provide pest control report from a licensed pest control company that indicates the type of control measures taken and that the facility has been treated. - Eradicate all live and dead rodents and insects from the facility. - Clean and sanitize the affected area(s) and equipment. - Dispose of all food items that have been adulterated/contaminated. - Seal holes (e.g. in walls and ceilings, repair coved base), and entryways (e.g. weather-stripping around doors); keep exterior doors closed; screen openable windows. - Eliminate harborage inside and outside of the facility - remove unused/cast-off equipment, contributory vegetation, etc.) - Eliminate food and water sources inside and outside the facility. - Properly store items (e.g. at least 6 inches above floor in sealed rodent-proof/insect-proof containers; organize overly packed and disorganized storage areas). - Properly store and dispose of garbage and other waste (e.g. bagged wastes, keep dumpster lids closed, frequent removal). - Subsequent follow-up inspection after first follow-up shall be billed $298/hour, minimum one hour, during normal business hours (Monday - Friday, 7:30 AM to 4:30 PM), and $645 for a minimum of two hours, during non-business hours, and upon inspector availability. - The posted placard is property of the Santa Clara County Department of Environmental Health and shall not be removed, covered-up, relocated, tampered, or copied.     Numerous large pots of stored outside in the back area. [CA] Equipment shall be stored within an approved facility to prevent contamination. Properly store utensils and equipment in a clean and sanitary manner.

  • K09: Proper cooling methods

    Observed two plastic containers of fried fish cake kept covered maintained inside the three-door merchandiser refrigerator measured at 59F. PIC confirmed with manager over the phone that items were cooked approximately an hour and thirty minutes prior. Red plastic container of cooked coagulated blood cubes kept out in ambient temperature measured at 83F. Manager over the phone stated that items were cooked about an hour prior. Multiple medium to large pots of soup maintained out in ambient temperature cooling, measured between 100F to 110F. Employees stated that soups were prepared approximately an hour and thirty minutes prior. Upon this Division entering the kitchen, employees observed scrambling and placed pots inside upright freezers and merchandiser refrigerator. [CA] Potentially Hazardous Foods (PHFs) shall be cooled rapidly from 135°F to 70°F within 2 hours and from 70°F to 41°F within 4 hours. Approved cooling methods include: 1) Placing the food in shallow pans 2) Separating the food into thinner or smaller portions. 3) Using rapid cooling equipment (Ex. blast chiller) 4) Using containers that facilitate heat transfer (ex. stainless steel) 5) Adding ice as an ingredient. 6) Using ice paddles 7) Using an ice bath and stirring frequently 8) Accordance with a HACCP plan. Cooling foods shall have enough space around the containers for cold air to circulate, be loosely covered, or uncovered and stirred as frequently needed to evenly cool. * Discontinue cooling large amounts of items inside the merchandiser refrigerator.
