The permit category for this facility is FP09. An invoice for the permit fee in the amount of $ 652.00 will be mailed to the billing address on the application. Payment must be submitted within 10 days of receipt of the invoice. The owner is responsible for contacting our department if an invoice is not received and remit payment within 30 days. The Environmental Health Permit will be effective: 2/1/2025 - 1/1/2026 This report serves as a temporary permit. However, the permit will be deemed invalid if the permit fee is not paid in full within 30 days from the date of this report. Okay to Operate. Missing hand wash sink in the back next to the 3-comp sink. Area seems to have a previously installed hand wash sink next to the 3-comp sink. A hand wash sink is available in the front but it's not easily accessible from the back. [CA] Install a handwash sink, if major plumbing or remodel is needed, submit plans to this department to install a handwash sink near the food prep area / warewashing area in the back.
