Employee at bar washed hands at 3 comp sink. [CA]: Employee shall wash hands at designated hand sink.

*REPEAT VIOLATION* Cove base in poor repair underneath the dish machine. [CA]: Repair cove base and maintain in good condition.
- K30: Food storage: food storage containers identified
*REPEAT VIOLATION* Food containers stored directly on the floor in walk in cooler and walk in freezer. [CA]: Food shall be stored at least 6 inches elevated from the floor on approved shelving that is easily cleanable (ex. Dunnage racks)

Small flies observed at ware-wash area and bar. [CA]: Remove small flies using safe, approved and effective methods. Ensure food and food contact surfaces are protected at all times from possible contamination. Remove any standing water in facility.

Food safety manager certificate not provided. [CA]: Provide proof of a valid food safety manager certification. Food facilities that prepare, handle, or serve non-prepackaged Potentially Hazardous Food shall have a valid food safety manager certificate available on site for review at all times *REPEAT VIOLATION* Food handler training certificates not provided. [CA]: Each food handler shall maintain a valid food handler card for the duration of his or her employment as food handler. A valid food handler card shall be provided within 30 days of after the date of hire.

*REPEAT VIOLATION* 1. Accumulation of black debris on the interior surface of ice machine. No direct contamination observed. [CA]: Properly clean and sanitize the interior of ice machine. Food contact surface equipment shall be maintained clean at all times. Ensure ice is protected at all times during cleaning. 2. Dish machine at kitchen dispensed <50 ppm of Chlorine sanitizer. No active ware-washing observed at the time. Available 2 comp ware-wash sink observed. [CA]: Dish machine shall be able to dispense 50 ppm of Chlorine sanitizer. Assess/repair/adjust as needed. Discontinue use until it is properly working.

In the steam table, prime rib measured at 80F. Per employee, prime rib was taken from the cooler due to a customer order and then placed in the steam table approximately less than 20 minutes prior to measurement. Employee stated it was supposed to be placed in the oven to be reheated prior to placing in steam table to hot hold. [CA]: Potentially Hazardous Food shall be hot held at 135F or above at all times. [SA]: Prime rib was placed into the oven to be reheated to 165F prior to hot holding in steam table at 135F or above.
