Follow up conducted following a closure for vermin on 9/4/2025. No evidence of vermin activity observed during this inspection. Facility is approved to reopen. Discussed with owner and manager to continue professional pest control service to continue to monitor for vermin and maintain the facility vermin free. Facility shall read and follow pest control service's recommendation. Discussed storing food in rodent-proof /insect-proof containers and to maintain facility free of clutter. Discussed maintaining a daily and monthly cleaning schedule to ensure facility is kept clean. Discussed maintaining hot potentially hazardous foods at 135F or higher. Facility shall use thermometer to measure food items on steam table. If facility would like to use TPHC, provide a list of food items that will use TPHC and standard operating procedures for TPHC; send via email. On site for a follow up inspection for routine inspection conducted on 9/4/25. Facility was closed due to vermin infestation. Facility will continue to remain closed due to cockroach and rodent activity. Facility still has excessive grease accumulation and standing water that needs cleaning. Maintain items six inches above the floor. Remove any unused equipment to prevent pest harborage. Seal any open holes, cracks and crevices, use weather stripping around doors to prevent pest harborage. Discard any food that may been contaminated. Facility shall cease and deist all food operations. Facility is closed due to cockroach and rodent activity. Food facility shall remain closed and complete the following requirements before reopening: 1. Email the signed and completed Reopening Checklist to the assigned inspector. 2. Submit a copy of the pest control report from a licensed provider Ensure the facility is cleaned and sanitized and there is no evidence of rodent activity or any dead or live cockroach activity in the facility.
