Follow up inspection conducted regarding closure on 07/16/2026 for vermin infestation. Facility shall remain closed due to evidence of a vermin (cockroach) infestation. - Facility is prohibited from all food service activities which include, but not limited to: preparation, service, handling, distribution, and sale. - Facility shall remain closed until vermin infestation is completely abated and authorization is provided by this Division. - This is the facility's first follow up inspection. Subsequent follow-up inspection shall be billed at $282/hour, minimum one hour, during normal business hour, Monday - Friday 7:30 AM to 4:30 PM, and $645 for a minimum two hours, during non-business hours, and upon inspector availability. - Permit Owner to contact Supervising Environmental Health Specialist Anjani S. via cellphone number provided or via email at anjani.sircar@deh.sccgov.org to possibly schedule a follow up inspection during after hours on Sunday, 07/19/26. Limited inspection conducted in conjunction with a complaint investigation CO0160261. Facility is closed due to active rodent and cockroach infestation. Facility shall cease and deist all food operations. Food facility shall remain closed and complete the following requirements before reopening: 1. Email the signed and completed Reopening Checklist to the assigned inspector. 2. Submit a copy of the pest control report from a licensed provider Ensure the facility is cleaned and sanitized and there is no evidence of cockroaches and rodent droppings in the facility. All the affected equipment shall be cleaned and sanitized. Any adulterated or contaminated food shall be discarded. Any open holes or gaps shall be sealed. On site for follow up inspection for routine inspection conducted on 6/26/25. Hand wash sink is operable with both cold and hot water. Soap is available at hand wash sink. Provide TPHC (time as public health control) logs for PHFs kept in small batches in prep unit in front of cookline daily to Environmental Health Specialist on the email address provided. Provide information whether the refrigeration until will be repaired or replaced by 7/10/2025 . A follow up will be conducted to ensure violation K07 and k36 are corrected. 2nd follow up inspection is a charged inspection.

PHFs such as cooked sauced, cooked vegetables and milk measured at 50-57F. Per PIC, the refrigerator is not working properly and all the PHFs were pulled out from walk in cooler 3 hours ago. [CA] PHFs shall be held at 41°F or below or at 135°F or above. [SA] PIC will maintain PHFs at TPHC and use or discard by the end of 4 hours from when it was temperature control.
