FOLLOW-UP INSPECTION

Follow-up inspection to routine inspection conducted on 8/27/2024 that resulted in the facility closure due to cockroach activity. A professional pest control treatment was obtained on 8/27 and 8/28 (invoices provided). Facility stated that a professional cleaning and sanitizing of the facility took place on 8/28. Observed more than 10 live cockroaches of various life stages throughout the facility. Facility shall remain closed due to continued vermin activity. When the facility is clear of all cockroach activity, call DEH at (408) 918-3400 to schedule a re-inspection. Any cockroach activity observed during the follow-up inspection will result in continued closure of the facility Continue to work on cleaning contaminated areas and repairing/ sealing vermin harborage points. The first follow-up inspection was free of charge. Facility will be billed $298.00 for the next follow-up inspection. Subsequent follow up inspections after first follow up will be billed $298/hr during business hours and $645/ hr during non business hours upon availability. Failure to comply may result in enforcement action.
