Hand wash stations are fully stocked and operational. - K14: Food contact surfaces clean, sanitized. 2-Compartment sink has been cleaned/sanitized properly. - K23: No rodents, insects, birds, or animals. No evidence of vermin activity within the facility at the time of inspection. Pest control was provided to specailist for review. The facility is herby closed by this department for vermin activity within facility. - Follow up inspection will be conducted on the 3-buiness days (6/11/25). - Contact Guillermo V. All major violations mentioned on inspection report must be addressed before follow up inspection. - Subsequent follow-up inspections after the first follow-up shall be billed $298-hour, minimum one hour, during normal business hours: Monday-Friday 7:30AM to 5:00PM or $645/hour, minimum one hour, during non-business hours, and upon inspector availability.

Observed Vermin activity: Documented in the following areas: - Rat droppings noted on top of shelving at the left side of the facility. - Behind reach in at the right side of facility. - Inside of the 2-compartment sink. - Inside of the restroom - On top of liquor boxes within the room with the 2-compartment sink. - Shelving with liquor has rat droppings and what looks like eaten cardboard around shelves. - Around the cashier station. 2. Photographs: Taken for documentation purposes. 3. Supervisor Notified: Monica Huato. 4. Notification: The person in charge during inspection, [Name], has been informed that the facility must close immediately. [CA]: The premises of each food facility must be maintained free of vermin. A facility cannot operate if there is a vermin infestation that leads to contamination of food contact surfaces, packaging, utensils, food equipment, or adulteration of food. The facility is required to cease operations immediately and must remain closed until all corrective actions on the provided checklist are completed. Requirements Before Reopening: 1. Email the signed and completed Reopening Checklist to Guillermo V. (408) 918-7114(guillermo.vazquez@deh.sccgov.org) cc the following email: DEHWEB@deh.sccgov.org 2. Submit a copy of the pest control report from a licensed provider.

    Noted that the front door of the facility is being kept open at the time of inspection. [CA] Doors closed at all times to prevent the entrance and harborage of vermin.

    Mold like substance is starting to grow within the silicon and around the mop sink area. [CA] Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris.

    Valves for the hot/cold water within the mop sink are damaged at the time of inspection. [CA] Repair plumbing fixture and maintain in clean and good repair.

    Rat droppings noted within the the 2-compartment sink at the time of inspection. Per PIC, they do not use the sink to wash/sanitize any equipment. All food sold is prepackage. [CA] Clean/sanitize the 2-compartment sink.
