    One dying cockroach covered in insecticide spray observed crawling on floor inside the restroom in the kitchen. Owner stated that pest control company was in the facility less than 30 minutes prior for service, just before this Division's arrival. Per owner, facility is serviced every other month. Pest control reports were provided for review. [CA] The premises of each food facility must be maintained free of vermin. A facility cannot operate if there is a vermin infestation that leads to contamination of food contact surfaces, packaging, utensils, food equipment, or adulteration of food. The facility is required to cease operations immediately and must remain closed until all corrective actions on the provided checklist are completed. [COS] Cockroach was abated.

    Restroom door observed maintained opened. [CA] Toilet rooms shall be separated by well-fitted, self-closing doors that prevent the passage of flies, dust, or odors.

    Manager observed using two-compartment ware wash sink to wash hands. [CA] Utilize sinks for their designated purpose. Hand washing shall be conducted only at the hand wash sink.

    One live cockroach was observed on floor below the two-compartment ware wash sink. Two live cockroaches was observed on sticky monitoring trap placed on floor behind the food preparation refrigerator. No other signs of live or dead cockroaches were observed. Per manager, facility is serviced monthly. Last treatment was conducted on 11/13/2024. Service report was provided for review. Report stated no findings. [CA] The premises of each food facility shall be kept free of vermin. A food facility shall not operate when there is a vermin infestation that has resulted in the contamination of food contact surfaces, food packaging, utensils, food equipment, or adulteration of food(s). The food facility shall cease operation of the food facility immediately. The food facility shall remain closed until: there is no longer evidence of a vermin infestation; all contaminated surfaces have been cleaned and sanitized; and contributing factors such as cleaning, repairs, and the elimination of harborages have been resolved. [COS] Manager abated live cockroach and removed monitoring trap.

    Two containers of marinara sauce maintained directly on the floor inside the walk-in refrigerator. [CA] Food shall be stored at least 6 inches above the floor to prevent contamination.

    The only hand wash station in the kitchen was partially obstructed with a garbage bag placed inside the wash basin. Manager stated that the bag was just placed there temporarily while he was preparing food. [CA] Handwashing facility shall be clean, unobstructed, and accessible at all times. [COS] Obstruction was removed.
