Follow up inspection conducted regarding closure on 08/04/26 and 08/06/26 for vermin (cockroach) infestation. Facility is ordered closed due to evidence of a vermin infestation. Facility is to immediately cease and desist all operations. - Facility is prohibited from all food service activities which include, but not limited to: preparation, service, handling, distribution, and sale. Facility shall remain closed until vermin infestation is completely abated and authorization is provided by this Division. -This is the facility's second follow up inspection and shall be charged. On this date german cockroach activity was still observed at the following locations: - One live adult cockroach on the floor between the three compartment sink and reach in freezer - One live adult cockroach on the wall behind the reach in freezer - Two live adults on the top panel of the ice machine - One live nymph in the left hinge of the ice machine reach in panel - One egg casing and one adult in the right hinge of the ice machine reach in panel - One dead adult on the floor between the upright coolers and walk in cooler - Spotting around the uncovered electrical socket behind/below the reach in freezer - Due to continued observed cockroach activity, facility shall remain closed - This is the first re-inspection.
