ROUTINE INSPECTION

Change of Ownership: Note: This limited/routine inspection is conducted for a change of ownership for SR0882568 NEW OWNER: Hong Li NEW FACILITY NAME: The Skewer Bar The applicant has completed the application for an Environmental Health Permit. FP11 $1,580.00 will be invoiced and mailed to the address on the application. Payment shall be paid within 10 days of receipt of the invoice. The owner is responsible for contacting our department if an invoice is not received within 30 days of the change of owner’s date. Permit issued and valid: 11/1/2025 - 10/31/2026. The permit will be deemed invalid if the permit fee is not paid in full within 30 days of the date of this report. This report serves as a temporary permit until the official permit is mailed to the facility. When the invoice for the facility is received, payment must be made within 10 days. Okay to operate as of today. The official Environmental Health permit must post permits from a public view and a copy of the most recent inspection report must be kept on file for review by anyone upon request. Someone from the facility must take a food manager certificate class within two months. Note: 1. This follow-up inspection is conducted before the official opening time of the restaurant. 2. This follow-up inspection is conducted for the routine inspection conducted on 09/10/2025. The facility was issued a conditional pass for the lack of supplies on the handwash stations and for employees failing to wash hand when it was needed. During today's follow-up inspection, both violations were observed to be corrected. Therefore, the facility is issued a green placard. However, the above repeated and new violations must be corrected immediately as well.

Note: This is a follow-up inspection to the routine inspection that was conducted on 11/24/25. The facility was closed due to cockroach infestation. During today's follow-up inspection, no live cockroach was observed in the kitchen. The mechanical dishwasher is pumping 50 ppm chlorine and the handwash stations have the soap and paper towel dispensers filled with supplies. Therefore, the facility is open for service. However, the above minor repeated violations must be corrected immediately as well.

The manager was demonstrated on the proper washing procedures. The dishwasher was demonstrated on the proper method of dishwashing. The utensils were rewashed accordingly (COS). Minor Violation: Equipment and utensils that are not currently in use are not clean.to sight and touch. Contamination of food-contact surfaces that could result in food contamination. [CA] A food facility shall not be operating if there is gross contamination of food- contact surfaces that may result in the contamination of food products. The food facility shall cease operation of the food facility until all food contact surfaces have been thoroughly cleaned and sanitized. There was a very low sanitizer concentration on the low temperature mechanical dishwasher. [CA] Whenever sanitizing of utensils/equipment is conducted, the sanitizer concentration maintain in the sanitizing compartment of the mechanical dishwasher must be maintained at 50 ppm chlorine unless the manufacture recommended otherwise.

Suzanne Lew Supervising Registered Environmental Health Specialist with the county of Santa Clara has approved the closure. Please contact the district inspector for follow-up inspection.

Some spices and condiments in an open container were stored below the table used to cut meat products. [CA] To prevent possible contamination, please do not store any food products on an open container below the preparation table and below sinks.

    There are gaps in between the screens of the exhaust hood. There also is an excessive dust, oil, and grease accumulation on the screens. [CA] To trap all the steam, smoke, grease, and heat produced by the equipment located below the exhaust hood, seal |the gaps observed between the screens. Clean the exhaust hood regularly.
