The juncture of the floor and wall shall be coved with a 3/8-inch minimum radius coving and shall extend up the wall at least 4 inches. Observed holes in FRP through dishwashing area. [CA] Repair holes in structure and maintain in good repair to prevent pest harborage.

    Paper towel dispenser at the only hand wash in preparation area was not working. [CA] Single-use sanitary towels shall be provided in working dispensers at all times during operation.

    Plugs were not available for one compartment of the three compartment sink. Second sink was set up for sanitizing. [CA] Plugs/stoppers shall be available to fill up the warewashing sink.

    Observed employee place large cutting board into mechanical warewash and run mechanical warewash. After cycle was complete chlorine measured 0ppm. Mechanical warewash was run several more times, liquid in bottle replaced and still measured 0ppm. [CA] Maintain chlorine in mechanical warewash at 50ppm. [SA] PIC set up two compartment sink to use for manual washing and sanitizing.
