ROUTINE INSPECTION
--48 HOUR NOTICE— Our records indicate this facility is operating without a valid permit and has a delinquent account. Your balance due is $1,383.75. Payment must be made immediately to DEH at the address at the top of this report or via our website at www.Ehinfo.org. A facility that submits payment of the outstanding balance within 48 hours (2 business days) of this notice will be issued and mailed an operating permit by this department. Failure to comply within 48 hours will result in facility closure and additional enforcement, including but not limited to reinspection(s) and additional cost recovery fees. A facility found to be in continued operation without a permit may incur a penalty of three (3) times the operating permit fee, and fee(s) for re-inspection(s) charged at the current hourly rate approved by the Santa Clara County Board of Supervisors. Penalties and fees incurred due to enforcement activities must be paid in full prior to the issuance of an operating permit. A follow up inspection will be conducted on 3/6/2026 for major violation k21.
Lack of cold water at hand wash sink near three compartment sink. [CA] An approved, adequate, protected, pressurized, potable supply of hot water and cold water shall be provided. [SA] Front hand wash sink will be used until cold water is repaired. A follow up inspection will be conducted on Friday 3/6/2026 to ensure compliance. Failure to comply may result in further enforcement actions.

ROUTINE INSPECTION
Lack of toilet paper in the restroom. [CA] Toilet tissue shall be provided in a permanently installed dispenser at each toilet.
Hot water measured at 117-118F at three compartment sink. [CA] Hot water shall be supplied at a minimum temperature of at least 120°F measured from the faucet throughout facility except at handwash sinks.
Shredded cheese measured at 44F in prep unit. [CA] PHFs shall be held at 41°F or below or at 135°F or above.
ROUTINE INSPECTION
Observed waste pipe extending in to the floor sink under food prep sink. [CA] All equipment that discharges waste shall be drained through a waste pipe with an airgap of at least one inch or twice the diameter of waste pipe into a floor sink or other approved type of receptor. [COS]
Measured sauces at 46F in under counter two door unit. Per PIC, sauces were stored in the refrigerator less than 2 hours ago. [CA] PHFs shall be held at 41°F or below or at 135°F or above. [COS] PIC moved the sauces to walk in cooler.
Observed mold like growth on sides of tray holder unit for bread defrosting. Per PIC, the unit will be removed from the facility. [CA] Equipment food-contact surfaces and utensils shall be clean to sight and touch. [COS] Owner removed the unit from the facility yo the trash area.
