On site for second follow up inspection. Facility was closed on 1/21/25 and 1/22/25 due to evidence of vermin infestation. No observations of live or dead cockroach activity in the facility. Per PIC, food preparation areas were cleaned and sanitized. Continue to clean and sanitize the floors. PIC provided pest control report from 1/23/25. Continue to seal any open holes and cracks to prevent vermin harborage. Continue to obtain professional pest control services as needed. This is a charged second follow up inspection. FOLLOW-UP INSPECTION

On site for follow up inspection. Facility was closed on 1/21/25 due to evidence of vermin infestation. On site, observed cockroaches in the following locations: - 2+ live nymphs on floor next to pizza prep station and hand wash sink. - 2 live nymphs on side wall of ventilation hood in the corner with hand wash sink. - 1 live nymph on ceiling where ceiling panel and ventilation hood meet - 1 live crawling on the wall next to hand wash sink Continue to obtain professional pest control activity as needed. Pest control report from 1/22/25 was provided by PIC. -Clean and sanitize the affected area(s) and equipment. Clean any grease accumulation on floors and walls near ventilation hood area. Facility shall continue to remain closed until all dead or live vermin activity is eliminated. Facility cannot re-open and must remain closed until the corrective action for K23 has been completed and a re-inspection has been conducted by this department. Facility is to cease and desist all food sales and operations, including food preparation, service, handling, distribution, and sale Ensure there is no evidence of live cockroach activity and there is not to be one live cockroach found during re-inspection or facility will remain closed. Subsequent follow-up inspection after first follow-up shall be billed $298/hour, minimum one hour, during normal business hours (Monday - Friday, 7:30 AM to 4:30 PM), and $645/hour, minimum two hours, during non-business hours, and upon inspector availability. - The posted placard is property of the Santa Clara County Department of Environmental Health and shall not be removed, covered-up, relocated, tampered, or copied. - On-site for a complaint investigation. - Due to imminent health hazard observed, a limited inspection was conducted. - See violation above. - Facility is hereby closed due to the evidence of a vermin infestation. - Facility is to immediately cease and desist all operations. - Facility is prohibited from all food service activities which include, but not limited to: preparation, service, handling, distribution, and sale. - Facility shall remain closed until vermin infestation is completely abated and authorization is provided by this Division. - Facility shall: - Provide pest control report from a licensed pest control company that indicates the type of control measures taken and that the facility has been treated. - Eradicate all live and dead rodents and insects from the facility. - Clean and sanitize the affected area(s) and equipment. - Dispose of all food items that have been adulterated/contaminated. - Seal holes (e.g. in walls and ceilings, repair coved base), and entryways (e.g. weather-stripping around doors); keep exterior doors closed; screen openable windows. - Eliminate harborage inside and outside of the facility - remove unused/cast-off equipment, contributory vegetation, etc.) - Eliminate food and water sources inside and outside the facility. - Properly store items (e.g. at least 6 inches above floor in sealed rodent-proof/insect-proof containers; organize overly packed and disorganized storage areas). - Properly store and dispose of garbage and other waste (e.g. bagged wastes, keep dumpster lids closed, frequent removal). - Subsequent follow-up inspection after first follow-up shall be billed $298/hour, minimum one hour, during normal business hours (Monday - Friday, 7:30 AM to 4:30 PM), and $645/hour, minimum two hours, during non-business hours, and upon inspector availability. - The posted placard is property of the Santa Clara County Department of Environmental Health and shall not be removed, covered-up, relocated, tampered, or copied. Facility is serviced monthly with the most recent treatment on 1/10/2025. Pest control report was provided for review. - Per pest report, activity was also found in the kitchen. [CA] The premises of each food facility shall be kept free of vermin.
