    Observed the usage of domestic equipment throughout facility including: air fryer, microwave, and blender. [CA] Discontinue the use of unapproved equipment. All equipment shall be certified or classified for sanitation by an American National Standards Institute (ANSI) accredited certification program (e.g. NSF, UL EPH, UL Sanitation, CSA Sanitation, or ETL Sanitation).

    Observed multiple foods stored in plastic grocery bags in refrigeration units. [CA] Food shall be stored in approved, food grade containers.

    Observed multiple scoop handles stored in direct contact with food, including in the ice machine and salt container. [CA] Store scoop handles above the food or in such a way to prevent direct hand contamination.

    In the dry food storage area, observed the ceiling panel is misaligned and creates a large gap in the ceiling. [CA] Seal all gaps in the facility. The facility shall be fully enclosed to prevent pest entrance and harborage.

    Facility lacks appropriate test strips for sanitizer of choice (chlorine). [CA] Testing equipment and materials shall be provided to adequately measure the applicable sanitization method used during manual or mechanical warewashing.

    Facility lacks probe thermometer for use. [CA] An accurate easily readable metal probe thermometer (accurate to +/- 2°F) that is designed to measure the temperature of thin masses shall be provided and readily available to accurately measure the temperatures of potentially hazardous foods.

    On the preparation counter, measured cooked bun, udon noodles, and pasta from 76-95F. Per employees, all noodles were cooked 1.5 hours prior to temperature measurement. [CA] Potentially Hazardous Foods shall be kept at 41F or below. [SA] PIC will discard the noodles at 13:00 (4 hours after initially cooked). Discussed TPHC procedures during inspection-- PIC decided that noodles will be properly cooled and kept under refrigeration rather than ambient temperature.

    In the kitchen, observed handwash sink is blocked by a trash can. A nearby handwash station is available for use. [CA] Handwash sink shall be fully stocked and accessible at all times. [COS] PIC relocated the trash can away from handwash sink.

ROUTINE INSPECTION

- Facility is hereby closed due to operating without a valid health permit. - 48-hour notice to pay was issued on 3/17/2025 via email to owner. - Multiple attempts made to conduct routine inspection and issue notice to pay: 12/16/24, 12/18/24, 12/31/24, 01/08/25, 01/24/25, 02/05/25. - Facility not operating at current posted times of operation. - Closure placard issued and posted on exterior window of facility. - Current inspection report provided as well. - Facility is to immediately cease and desist all operations. - Facility is prohibited from all food service activities which include, but not limited to: preparation, service, handling, distribution, and sale. - Current permit holders are to make payments for delinquent health permit or provide written notification of closure of business. - If new business owners, submit application for Facility Evaluation Request. *** The posted placard is property of the Santa Clara County Department of Environmental Health and shall not be removed, covered-up, relocated, tampered, or copied. Failure to comply may result in enforcement actions per County Ordinance Code Section B11-55. *** A facility found to be in continued operation without a permit may incur a penalty of three (3) times the operating permit fee, and fee(s) for re-inspection(s) charged at the current hourly rate approved by the Santa Clara County Board of Supervisors. Penalties and fees incurred due to enforcement activities must be paid in full prior to the issuance of an operating permit.

[CA] A food facility shall not be open for business without a valid health permit. CEASE AND DESIST ALL ACTIVITIES. A permit shall be issued by this department when investigation has determined that the proposed facility and its method of operation meets the specifications of the approved plans or conforms to the requirements set forth by this department.
