    Observed the use of duct tape to seal a gap between the dump sink and the warewash machine. [CA] Ensure only approved, smooth, durable, and easily cleanable materials/equipment are used to prevent possible harborage of vermin.

    At the side bar, observed an open/exposed outlet installed without an outlet cover. At the side prep station, observed wooden flooring installed. [CA] Ensure all cracks and crevices are sealed to prevent possible harborage of vermin. Areas with food preparation shall have proper flooring installed that is smooth, durable, and easily cleanable.

    In the bar area, and in the back storage room, observed several fruit flies. [CA] Ensure facility is kept free of fruit flies to prevent possible contamination of food.

  • K14: Food contact surfaces clean, sanitized

    At the bar, in the warewash machine, measured chlorine sanitizer at 25 PPM. [CA] Ensure chlorine sanitizer at the warewash machine is maintained at 50 PPM to allow for proper cleaning and sanitizing of utensils and equipment.

ROUTINE INSPECTION

OWNERSHIP CHANGE INFORMATION NEW FACILITY NAME: SECOND STORY NEW OWNER: DJJ LG15 LLC The applicant has completed the facility evaluation application process for an Environmental Health Permit. The permit category for this facility is FP12. An invoice for the permit fee in the amount of $896.00 will be mailed to the billing address on the application. Payment must be submitted within 10 days of receipt of the invoice. The owner is responsible for contacting our department if an invoice is not received and remit payment within 30 days. The Environmental Health Permit will be effective: 12/01/2024 - 11/30/2025 This report serves as a temporary permit. However, the permit will be deemed invalid if the permit fee is not paid in full within 30 days from the date of this report. Okay to Operate. An official permit will be mailed to the address on file and shall be posted in public view upon receipt. *Structural Review inspection conducted on 11/13/2024 *Permit condition: n/a *Obtain food safety manager certificate within 60 days. All other food employees must have valid food handler cards within 30 days from hire date.

    Observed unused and old bottles of alcohol stored in the dry storage room. Per operator, alcohol is left from previous ownership and all open bottles are to be discarded. Noted fruit flies in several old bottles of alcohol. [CA] Ensure garbage and refuse is removed the facility frequently to prevent any form of nuisance.
