Observed brown mold like substance on inside panels and well of ice machine. [CA] Thoroughly clean and sanitize on a regular basis.

Observed potentially hazardous food that measured in the temperature danger zone located in the warmer (ground pork 124F and cooked beef 124F). Per PIC, the meat was cooked in the morning around 11am. [CA] Ensure PHFs are properly cold held at 41F or below OR hot held at 135F or above. Repair, adjust or replace warmer if necessary. [SA] PIC stated lunch service is over at 2pm and he will discard left over meat.
