Ecoguard Pest Management conducted inspection of this facility on 08/07/2026 and did not observe signs of rodents. Facility appeared to have been cleaned. Plates and cooking utensils were all moved into the dining room. Refrigerators were pulled out for cleaning around the back. Per owner, rodent droppings were removed and surfaces were sanitized. No rodents or rodent droppings were observed at time of this inspection. Sticky traps were placed around this facility and all were found to be clear. Observed opening next to electrical outlet (next to kitchen window) was sealed with wire mesh. Opening in the exterior wall (facing parking lot) was sealed. Opened bags of bulk dry ingredients were observed. Employee transferred the product to a hard container with lid. Health permit to be reinstated and facility is okay to operate. Per owner, this facility is serviced by Ecoguard Pest Management twice a month. If there are any signs of rodents, call pest control company immediately. Prior to reopening, ensure all plates and cooking utensils are cleaned and sanitized. Bottle of Chlorox was available. Leak was observed under the 3-compartment sink. Repair the leak to eliminate water source.

FOLLOW-UP INSPECTION

This is the first follow up inspection where the facility was closed during an onsite commissary evaluation for an active vermin infestation. A pest control report and checklist was provided prior to this inspection. Observations: K23: No evidence of rodents were observed. Gap near the ice machine and in the electronics area have been repaired. Facility has cleaned the produce rack area. A potential entry was observed inside the cabinet of the restroom sink. Staff sealed the gap at time of the inspection. Note: Pest control report did not comment on activity of vermin. When facility operator called their pest control company they denied to connect to the technician for additional information when informed that DEH staff were present. Provide an additional pest control report by 10/21/2025. Facility is okay to reopen. An additional follow up inspection will be conducted to verify continued compliance. ________ Onsite for a commissary evaluation for El Conrro Lina B #39355A4. All services are available. Facility can also provide warewash facilities, supply food/ice, and store chemicals. Facility will only have El Conrro Lina B #39355A4 and Super Tacos El Conrro #3 4UA6837 parking onsite. [CA] Clean and sanitize area of dead cockroaches or old droppings. 5. In the kitchen area, above a conduit pipe, a dead cockroach and a live adult cockroach were observed. Staff tried to eliminate it but it went back into the wall. 6. In the kitchen area, above the three compartment sink by a metal shelf, a nymph was observed to crawl in and out of a hole in the wall. Staff tried to eliminate it but it got away. 7. In the kitchen area, on the wall near the fire extinguisher, two german cockroaches were observed in a small hole in the wall. [CA] The premises of each food facility shall be kept free of vermin. A food facility shall not operate when there is a vermin infestation that has resulted in the contamination of food contact surfaces, food packaging, utensils, food equipment, or adulteration of food(s). The food facility shall cease operation of the food facility immediately. The food facility shall remain closed until: there is no longer evidence of a vermin infestation; all contaminated surfaces have been cleaned and sanitized; and contributing factors such as cleaning, repairs, and the elimination of harborages have been resolved. Contact District Specialist at (408) 497-0913 to schedule follow up inspection once the facility has been serviced and cleaned. At time of the follow up inspection the infestation shall be abated, no live or dead cockroaches shall be observed.
