Routine inspection conducted in conjunction with CO0160416. A follow-up inspection will be conducted within one business day to verify compliance of violation K23. The first follow-up inspection is free of charge. Subsequent follow-up inspections after the first follow-up shall be billed at $282/hour (one hour minimum) during normal business hours (Monday - Friday, 7:30 AM to 4:30 PM), and $645 (minimum of two hours) during non-business hours, and upon inspector availability.

    Observed employee directly discarding foods into the trash can, removing gloves, then donning new gloves to handle clean equipment without an intermediate handwashing step. [CA] Employees are required to wash their hands: 1) before beginning work, 2) before handling food or clean equipment, 3) as often as necessary during food preparation to remove soil and contamination, 4) when switching from raw to ready to eat foods, 5) after touching body parts, 6) after coughing, sneezing, eating, or drinking, 7) after using the restroom, and 8) any time when contamination may occur. [COS] Specialist intervened and directed employee to properly wash hands prior to donning gloves and handling clean equipment.

The premises of a food facility shall be maintained free of vermin. 2) Observed a couple of drain flies underneath the mechanical dishwasher. [CA] The premises of a food facility shall be maintained free of other non-disease carrying insects, such as drain flies. NOTE: Reviewed last pest control services report from Habitat Pest Control on 6/27/2026. Per report, no rodent activity was observed at time of inspection. PIC stated that facility will continue to receive monthly pest control services.

    Measured chlorine sanitizer concentration in the mechanical dishwasher at 25 ppm. [CA] Mechanical sanitization shall be accomplished in the final sanitizing rinse with a chlorine concentration of 50 ppm, or as per manufacturer's specifications.

    On preparation counter next to cook line, measured fries from 41 to 47F. Per employees, fries were removed from refrigeration unit less than 20 minutes prior to temperature measurement. [CA] Potentially Hazardous Foods shall be kept at 41F or below or 135F or above. [COS] Fries were relocated to the walk-in cooler to facilitate rapid cooling.

On-site for a follow-up inspection after facility earned a conditional pass during the last routine inspection conducted on 02/05/2026. Compliance of the following has been verified: K07: Improper hot and cold holding temperatures - All potentially hazardous foods measured at 41F or below, or at 135F or above (except for those being actively prepared) K09: Improper cooling methods - No active cooling observed. Per staff, the policy is to rapidly cool down in the blast chiller, or else maintain uncovered in shallow pans in the walk-in cooler. K23: Observed rodent activity - No evidence of rodents observed. Continue to work with a licensed pest control agency. Facility re-issued a Green "Pass". Continue to work on the violation cited during the last routine inspection.

[CA] Frozen potentially hazardous food shall only be thawed in one of the following ways: 1) under refrigeration that maintains the food temperature at 41°F or below, 2) completely submerged under potable running water for a period not to exceed two hours at a water temperature of 70°F or below, and with sufficient water velocity to agitate and flush off loose particles into the sink drain, 3) in a microwave oven if immediately followed by immediate preparation, 4) as part of a cooking process.

    Several containers of brisket measured 49F while maintained in deep containers kept tightly wrapped with plastic and aluminum foil on a shelf in the right-side walk-in refrigerator. Per the Head Manager, and per labels on the containers, brisket had been cooked between 2 and 3 days prior, and had been cooling while maintained in the walk-in refrigerator since. [CA] After heating or hot holding, Potentially Hazardous Foods (PHFs) shall be cooled rapidly from 135°F to 70°F within 2 hours and from 70°F to 41°F within 4 hours. Approved cooling methods include: 1) Placing the food in shallow pans 2) Separating the food into thinner or smaller portions. 3) Using rapid cooling equipment (Ex. blast chiller) 4) Using containers that facilitate heat transfer (ex. stainless steel) 5) Adding ice as an ingredient. 6) Using ice paddles 7) Using an ice bath and stirring frequently 8) Accordance with a HACCP plan. Cooling foods shall have enough space around the containers for cold air to circulate, be loosely covered, or uncovered and stirred as frequently needed to evenly cool. Raw chicken measured 51F while maintained in a container of standing water on the drainboard of the preparation sink. Per staff, it had been taken out of the walk-in refrigerator two hours prior for thawing. [Minor thawing violation leading to Major holding violation] Brisket measured 120F while maintained in the insert second-to-the-right in the steam table at the entrance of the main preparation area. Per staff, it had been cooked 1.5 hours prior. *Note: Recommend covering steam table with a lid if it allows it to maintain at 135F or above A tray of cooked, bone-in chicken measured between 120F and 127F while maintained in the upright warmer at the entrance of the main preparation area. Per staff, it had been cooked and placed into the upright warmer approximately 4 hours prior. *Note: Tray of chicken was the only potentially hazardous food in the upright warmer not covered with aluminum foil. Several other potentially hazardous foods measured at 135F while maintained in the upright warmer. Recommend covering chicken with aluminum foil if that allows it to maintain at 135F or above. [CA] Potentially hazardous foods shall be held at 41F or below, or at 135F or above to prevent the growth of bacteria. [COS] Brisket burnt ends relocated to preparation refrigerator. Brisket in the steam table reheated to 165F in the oven, placed back into hot holding, and then covered with a lid. Tray of bone-in chicken voluntarily discarded into the trash.

This was a follow-up to the 11/24/25 limited inspection. Owner had provided specialist with a pest control report and completed reopening checklist. No rodent droppings were observed today. Facility had repaired all major holes/damage in walls, including in the restroom where the pipe had been leaking. No notable sewage smell was present today. Facility was re-opened. Limited inspection conducted in conjunction with CO0158407 Facility is hereby closed owing to evidence of a rodent infestation. Facility is to immediately cease and desist all operations, including but not limited to food preparation, service, handling, distribution, and sale Facility must provide the following prior to reopening: 1) A pest control report from a certified pest control agency stating that treatment was performed, and that no remaining vermin activity was observed 2) The filled out “Vermin Reopening Checklist” provided by this division Email documents to mindy.nguyen@deh.sccgov.org .
