3nd follow inspection conducted after limited inspection conducted on 6/4, 1st follow up inspection conducted on 6/6 and 2nd follow up inspection conducted on 6/7. No dead or live cockroach activity observed in the facility. Cardboard boxes have been moved on racks in the back storage area. Facility is ok to reopen. Note: This is a charged inspection. An invoice will be mailed to the facility.

FOLLOW-UP INSPECTION

Note: This is a follow-up inspection to the complaint investigation conducted on . The facility was closed on 06/06/2025 due to vermin infestation. The checklist submitted to our office on 06/07/2025 states that the facility does not have any live and dead cockroaches. During the time of this inspection multiple dead and single dying cockroach was observed below the utensils storage shelves. Many piles of boxes were also stored on the floor which might be used a hiding area for vermin. Therefore, the facility shall remain until all all live or dead cockroaches are removed and all possible harborage areas are eliminated and cleaned. 2. Please contact our office at 408 918 3400 for a third follow-up inspection. Any follow-up inspections shall be conducted at a charge of $298.00/hour to the owner. 3. If any live or dead cockroaches are found or if any of the structures which contribute for vermin hiding or harboarge are not addressed during the next follow-up inspection, the owner of the business will be called for office hearing.

Follow up inspection for routine inspection conducted on 6/4/25. Vermin Checklist and Pest Control Report were emailed by the owner. Facility shall remain closed until there is no vermin activity in the facility. Facility shall cease and deist all food operations. Food facility shall remain closed and complete the following requirements before reopening: 1. Email the signed and completed Reopening Checklist to the assigned inspector. 2. Submit a copy of the pest control report from a licensed provider. Red Placard was posted on 6/4/25 during limited inspection. Ensure the facility is cleaned and sanitized and there is no evidence of dead or live cockroach activity in the facility. All the affected equipment shall be cleaned and sanitized. - Subsequent follow-up inspection after first follow-up shall be billed $298/hour, minimum one hour, during normal business hours (Monday - Friday, 7:30 AM to 4:30 PM), and $645/hour, minimum two hours, during non-business hours, and upon inspector availability. - The posted placard is property of the Santa Clara County Department of Environmental Health and shall not be removed, covered-up, relocated, tampered, or copied. Follow up inspection conducted for routine inspection conducted on 1/22. No live cockroach activity observed in the facility. Observed dead cockroaches in cabinet with wiring under cash register. Areas with dead cockroaches from last inspection were cleaned. Clean and sanitize areas of dead cockroaches. Per owner, pest control is scheduled for today. Owner provided food handler card copies. Ensure two door refrigerator maintains PHFs at 41F or below.
