This was an unannounced follow-up/routine inspection after the 7/01/2025 office hearing to discuss violations regarding handsinks, food temperatures, and vermin. Observed major violation in temperature holding. Due to the observation of a repeat violation (food holding temperatures), further enforcement action will occur. Inspector will follow-up with facility/owner. Due to technical difficulties, this report will be emailed to the facility.

FOLLOW-UP INSPECTION

This was a follow-up to the 6/23/25 routine inspection. The major violations were observed to be in compliance as follows: - No evidence of cockroaches were observed. - Rear handsink was functional and was able to dispense water. - PHFs measured below 41F in the walk-in refrigerator. Facility had provided a completed Vermin Reopening Checklist and a professional pest control report indicating that no evidence of cockroaches observed. Facility was OK to reopen. Continue working closely with pest control, deep clean all surfaces (especially in cooking equipment area), seal all cracks/crevices in equipment/walls/floors/ceilings. Provided office hearing letter to owner on site. Joint inspection with Maverick Chin. Notified DEH supervisor of reopening.

ROUTINE INSPECTION

FACILITY WAS CLOSED DUE TO EVIDENCE OF AN ACTIVE COCKROACH INFESTATION AND LACK OF FUNCTIONAL HANDSINK WATER SUPPLY IN THE REAR KITCHEN AREA. Note: the handsink was repaired at the end of the inspection. PIC was aware of the cockroach issue and stated there was a recent sewage backup earlier this month that had already been resolved, but appeared to be to cause of the cockroach problem. No evidence of sewage backup today after sinks were run and toilets were flushed. Checklist for Reopening after Closure Due to Vermin Infestation was provided to the facility. When the checklist has been completed and signed, email it to mindy.nguyen@deh.sccgov.org and call (408) 918-3400 to schedule a reopening. The first follow-up inspection is free of charge. Any subsequent follow-up inspections will be billed $298/hr during business hours, or $645 during non-business hours upon availability. Per the previous inspection reports, due to the repeat major violations regarding lack of adequate handwash facilities, there will be an office hearing next week. This was a follow-up to the 1/30/25 limited inspection. 1) Observed both kitchen handwash sinks were functional, unobstructed, and their soap and paper towel dispensers were stocked. 2) Observed minor temperature violations for the foods inside the front prep unit. Note: the front display thermometer of the front prep unit showed "41F." Ensure unit is repaired and that the temperature display is accurate. Due to the lack of major violations today, the yellow placard was replaced with a green placard. Ok to operate.

    1) At the rear main kitchen handwash sink (operated via floor foot pedals), observed the following: - Both the soap and paper towel dispensers were empty. - Multiple large plastic bottles were stored directly in front of the foot pedals, making them inaccessible. - When inspector asked employees to remove the plastic bottles, the pedals did not turn on the water. - When inspector asked employees to try turning on the water, one employee reached behind the handwash sink and turned a valve to turn on the water. This was observed to be the only way to turn on water. 2) The front handwash sink in the side dish prep area was clogged. Employees stated they are not using it due to it being clogged. Observed standing water inside the sink. [CA] Handwash sinks must be easily accessible and functional at all times. Soap and paper towel dispensers must be stocked at all times. [SA] A plumber was called and began repairs on both sinks. The front sink was repaired during the inspection, and the rear sink was almost fully fixed (pedals were already repaired).
