This is the first follow up inspection and shall be charged at the approved hourly rate. A checklist for reopening and a pest control report confirming no pest activity was provided prior to this inspection. Observations: Facility shall remain closed. - K23: No rodents, insects, birds, or animals. Evidence of vermin noted within the facility at the time of inspection. Noted within the following areas: - Three live cockroaches under the counter gap where the prep cooler was previously installed. - A live cockroach was observed coming out the cove base under the menu cabinets and quickly went back in. - A live nymph in the cabinet under the phone service area, on cardbaord under the printer. Facility is to remain closed and not allowed to re-open until written authorization is given by this department. - Facility shall: * Eradicate all live and dead rodents and insects from the facility. * Clean and sanitize the affected area(s) and equipment. * Dispose of all food items that have been adulterated/contaminated. * Seal holes (e.g. in walls and ceilings, repair coved base), and entryways (e.g. weather-stripping around doors); keep exterior doors closed; screen openable windows. - Subsequent follow-up inspection shall be billed at $282/hour, minimum one hour, during normal business hour, Monday - Friday 7:30 AM to 4:30 PM, and $645 for a minimum of two hours, during non-business hours, and upon inspector availability. ROUTINE INSPECTION

The facility is hereby CLOSED by this department due to evidence of vermin infestation (see violation K23). The facility is required to cease operations immediately and must remain closed until all corrective actions on the provided checklist are completed. Requirements Before Reopening: 1. Complete and sign the provided re-opening checklist. 2. Obtain a copy of the latest pest control report from a licensed provider. 3. Submit the re-opening checklist and latest pest control report to district specialist (kathy.vo@deh.sccgov.org) or contact DEH main line (408-918-3400). Upon review, district specialist will schedule the follow-up inspection. Facility receives monthly pest control services. Reviewed last pest control services report dated 2/4/2026 from Clark Pest Control. Report noted no activity observed at time of inspection. Per PIC, facility received services earlier this month, but is unable to locate pest control report. NOTE: A period of 24 hours from this inspection shall pass before requesting a follow-up inspection. Pest control treatments may require a sufficient amount of contact time to be effective. - A follow-up inspection to verify compliance of major violations will be conducted upon facility request. The first follow-up inspection is free of charge. Subsequent follow-up inspections after the first follow-up shall be billed at $298/hour (one hour minimum) during normal business hours (Monday - Friday, 7:30 AM to 4:30 PM), and $645 (minimum of two hours) during non-business hours, and upon inspector availability. - The posted placard is property of the Santa Clara County Department of Environmental Health and shall not be removed, covered-up, relocated, tampered, or copied. (After heating or hot holding, Potentially Hazardous Foods (PHFs) shall be cooled rapidly from 135°F to 70°F within 2 hours and from 70°F to 41°F within 4 hours.) * Facility shall provide an updated SOP which indicates how pizzas measuring above 70°F after the 2 hour period shall be handled.
