Observed roll of paper towel not stored in the paper towel dispenser. [Corrective Action] Ensure paper towel is stored in the paper towel dispenser. [Corrected on Site] PIC stored the roll of paper towel in the dispenser.

The self-closing device on the restroom door was observed detached. [Corrective Action] Re-attach the self-closing device on the restroom door.

The handwash sink in the kitchen was partially blocked by a plastic container in the basin. [Corrective Action] Do not store anything in the handwash sink. The handwash sink shall be easily accessible at all times.

The self-closing device on the restroom door is broken. [Corrective Action] Repair or replace the self-closing device on the restroom door.

1. Black mold like substance observed on the wall around the 3 compartment sink. [Corrective Action] Thoroughly clean and maintain clean. 2. Paint peeling from the wall at the 3 compartment sink. [Corrective Action] Maintain wall in good condition.

Heavy dust accumulation on the fan guard in the restroom. [Corrective Action] Thoroughly clean and maintain clean.
