ROUTINE INSPECTION
The facility is herby closed by this department with regards to the following major violation. - K21: Hot and cold water available. No hot water within the facility at the time of inspection. Subsequent follow-up inspections after the first follow-up shall be billed at $298/hour during normal business hours (Monday - Friday, 7:30 AM to 4:30 PM), and $645/hour during non-business hours, and upon inspector availability. The posted placard is property of the Santa Clara County Department of Environmental Health and shall not be removed, covered-up, relocated, tampered, or copied. Failure to comply may result in enforcement action per County Ordinance Code Section B11-55. A facility found to be in continued operation without a permit may incur a penalty of three (3) times the operating permit fee, and fee(s) for re-inspection(s) charged at the current hourly rate approved by the Santa Clara County Board of Supervisors. Penalties and fees incurred due to enforcement activities must be paid in full prior to the issuance of an operating permit. Notify this department once issues have been addressed. Flies noted in the back storage room of the facility. [CA] Food facility shall be kept free of non-disease carrying insects, weevils, ants, gnats, and fruit flies.

Some dishes are being washed within the hand wash station. No active ware-washing at the time of inspection. [CA] All equipment must be washed properly within the 3-compartment sink. Equipment must be wash, rinse, sanitized within 4-hours.
