Pest control were on-site earlier this morning and provided documentation that stated that no live or dead cockroaches were observed. Per discussion with pest control technician, Matthew, he stated that the infestation had been eliminated. 2) Signed and completed vermin reopening checklist During the follow-up inspection, live cockroach activity was observed. See violation above. *Facility will remain closed due to continued evidence of a cockroach infestation* Facility is to immediately cease and desist all operations, including but not limited to food preparation, service, handling, distribution, and sale. The following must again be provided prior to reopening: 1. The signed and completed Reopening Checklist 2. A copy of the pest control report from a licensed provider, stating that facility has been treated and that all vermin activity has been abated 3. Paid invoice for chargeable, second follow-up inspection A follow-up inspection must be conducted prior to facility reopening. A period of no less than 24 hours must pass prior to the follow-up inspection being conducted. Call or email to schedule.     *REPEAT VIOLATION* Imitation crab measured between 44F and 46F while maintained in an improper ice bath at the front service counter. Per the owner, imitation crab removed from refrigeration less than 30 minutes prior. Cooked chicken measured between 132F and 146F while maintained in a metal container on the stove with the burner underneath turned off. Per staff, chicken had been cooked less than 20 minutes prior. [CA] Potentially hazardous foods shall be held at 41F or below, or at 135F or above to prevent the growth of bacteria. [COS] Imitation crab and chicken were relocated to refrigeration.

On-site to conduct a follow-up to the previous routine inspection (dated 10/29/2025). The following major violations have been corrected: - K05: Hands clean and properly washed. Observed proper handwashing from employees prior to handling foods during inspection. - K06: Adequate handwash facilities supplied. Observed both handwash stations in the kitchen and front sushi preparation area are fully stocked with soap and paper towels. - K07: Proper holding temperatures. Measured all Potentially Hazardous Foods within proper holding temperatures. See measured observations. --- Facility now uses TPHC for sushi rice and California rolls. Observed proper time markings for sushi rice during inspection. No California rolls prepared at the time of inspection. Facility has earned a "PASS" for this inspection. Continue to address remaining violations as cited on the routine inspection report.
