    Various potentially hazardous foods, including uncanned olives, blue cheese dressing, ranch dressing, uncanned artichokes, chickpeas, cheese, mac salad, and potato salad measured between 44F and 46F while maintained in the countertop cold-hold inserts at the front service line. Per the owner, items had been relocated from the walk-in cooler to the front service line 1 hour prior. [CA] Potentially hazardous foods shall be held at 41F or below, or at 135F or above to prevent the growth of bacteria. [COS] All PHF"s in the unit to be served or else discarded by 6:45PM (4 hours elapsed after initially being removed from the walk-in cooler). *Note: If refrigeration at the front service line is unable to maintain potentially hazardous foods at 41F or below, recommend subjecting all potentially hazardous food items (raw or cooked animal products, heat treated vegetables, sliced tomatoes, etc.) to time as a public health control. When time only, rather than time and temperature is used as a public health control, PHFs shall be time marked to indicate when item is removed from temperature control, discarded if not consumed or served within 4 hours, and written procedures shall be readily available for review. OWNERSHIP CHANGE INFORMATION NEW FACILITY NAME: MOUNTAIN MIKE'S PIZZA NEW OWNER: EILRO INC. The applicant has completed the facility evaluation application process for an Environmental Health Permit. The permit category for this facility is FP10. An invoice for the permit fee in the amount of $1,107.00 will be mailed to the billing address on the application. Payment must be submitted within 10 days of receipt of the invoice. The owner is responsible for contacting our department if an invoice is not received and remit payment within 30 days. The Environmental Health Permit will be effective: 07/01/2025 - 06/30/2026 This report serves as a temporary permit. However, the permit will be deemed invalid if the permit fee is not paid in full within 30 days from the date of this report. Okay to Operate. An official permit will be mailed to the address on file and shall be posted in public view upon receipt. *Structural Review inspection conducted on 06/03/2025 *Permit condition: n/a *Obtain food safety manager certificate within 60 days. [SA] Operator to serve potentially hazardous food items in the salad bar by 1:30pm, for a total of 4 hours from when removed from temperature control or discard.
