    Observed food accumulation and debris on the floor under and behind most all equipment. The floor around the tall black shelf at the entrance of the sushi area has accumulated a thick layer of food particles. {CA] Thoroughly clean the floor mostly under equipment.

    Observed a dead cockroach on the floor in front of the kitchen hand wash sink. Observed the glue board to be filled with cockroaches and 1 live one. [CA] Take effective measures to eliminate the cockroaches. Protect food from contamination. Discard the full glue boards and provide new ones. Continue with the pest control service. Covering all holes on walls and caulking around wall mounted equipment will help eliminate insect harborage areas. A follow inspection will be made for correction of the cockroach issues.
