PHF items on the counter measured between 65F to 75F at the front service counter. PHF items maintained on the service line measured between 85F to 106F. ** Note: Time as a public health control (TPHC) has been revoked from facility for repeated non-compliance. Owner stated that food items were maintained on temperature control, however, the thermostat was turned off towards the end of the day, approximately one hour prior. PHF items maintained out in ambient temperature in the kitchen: fried vegetarian "meats," various types of vermicelli and udon noodles, soups. Items measured at 69F, 70F and between 103F to 110F respectively. Owner stated that items were prepared 2 hours prior with small portions at a time. - On-site for operator scheduled follow-up inspection after facility was closed during a follow-up inspection on 10/2/2025. - Current inspection will be charged at the hourly rate approved by the Santa Clara COunty Board of Supervisors. - Facility has conducted the following: - Obtained pest control services and provided reports to this Division for review. - Cleaned and sanitized all affected areas of vermin. - Sealed all gaps, cracks, and crevices in the kitchen to prevent harborage. - At time of follow-up inspection, no signs of vermin activity was observed. - Since facility was closed during time of follow-up inspection, all other major violations were discussed with PIC. - Continue to clean and maintain facility free of vermin. - Facility is hereby authorized to reopen and resume operations.

- On-site for follow-up inspection after facility earned a conditional pass during a routine inspection on 9/30/2025. - Facility is hereby closed due to evidence of a vermin infestation. - Facility is to immediately cease and desist all operations. - Facility is prohibited from all food service activities which include, but not limited to: preparation, service, handling, distribution, and sale. - Facility shall remain closed until vermin infestation is completely abated and authorization is provided by this Division. - Facility shall: - Provide pest control report from a licensed pest control company that indicates the type of control measures taken and that the facility has been treated. - Eradicate all live and dead rodents and insects from the facility. - Clean and sanitize the affected area(s) and equipment. - Dispose of all food items that have been adulterated/contaminated. - Seal holes (e.g. in walls and ceilings, repair coved base), and entryways (e.g. weather-stripping around doors); keep exterior doors closed; screen openable windows. - Eliminate harborage inside and outside of the facility - remove unused/cast-off equipment, contributory vegetation, etc.) - Eliminate food and water sources inside and outside the facility. - Properly store items (e.g. at least 6 inches above floor in sealed rodent-proof/insect-proof containers; organize overly packed and disorganized storage areas). - Properly store and dispose of garbage and other waste (e.g. bagged wastes, keep dumpster lids closed, frequent removal). ** Please note that pest control treatments for rodents or insects may require a sufficient amount of contact time for these to be effective. Manager stated that the items will be used for food preparation as soon as the current employee is finished prepping other items. [CA] PHFs shall be held at 41°F or below or at 135°F or above. Use only small portions of food during preparation so that it does not exceed the maximum amount of time allowed for diligent prep. [COS] Items prepared in the morning were relocated into the walk-in refrigerator to immediately cool.

Pest control report was provided for review upon request. No contamination of food or food contact surfaces were observed. No live cockroach activity was observed. Employee and owner stated that items were brought in from a customer and/or were supplied from a temple in Santa Clara County. Employees did not provide address or contact information for vendors. No invoices were provided. [CA] Food must be from an approved source or properly documented. [COS] All food items were discarded into the trash.
