- K07: Proper hot and cold holding temperatures
Numerous PHF items maintained at the top inserts of the food preparation refrigerator measured at 41F at the bottom to 45F the top. PHF items were maintained above the load limit of the refrigerator. [CA] PHFs shall be held at 41°F or below or at 135°F or above. Discontinue storing PHF items above the load limit.
- K33: Nonfood contact surfaces clean
Heavy accumulation of food residue observed on refrigerator doors, door handles, and gaskets. [CA] Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris.
- K06: Adequate handwash facilities supplied, accessible
Hand wash station in the first restroom lacked paper towels. Second hand wash station in separate restroom available. [CA] Handwashing facilities shall be equipped with handwashing cleanser and single-use sanitary towels in dispensers. Ensure hand soap and paper towels are easily accessible and readily available at all times. [COS] Employee refilled dispenser with towels.
- K08: Time as a public health control; procedures & records
Container of grilled meats subjected to time as a public health control (TPHC) were not labeled. All other items subjected to TPHC were properly labeled. Employee stated that they had ran out of labels. [CA] When time only, rather than time and temperature is used as a public health control, PHFs shall be time marked to indicate when item is removed from temperature control, discarded if not consumed or served within 4 hours, and written procedures shall be readily available for review. [COS] Items were VC&D by employee.

Observed gas powered rice cooker maintained by the water heater. [CA] Provide approved exhaust hood ventilation over all cooking equipment as required to effectively remove cooking odors, smoke, steam, grease, heat, and vapors. Relocate rice cooker.

Observed two live German cockroaches inside the cracked grout by the coved base tile to the left of the two-door upright freezer by the rear of the facility. Per manager, facility is serviced monthly with the last treatment on 3/21/2026. Pest control report was provided for review upon request. [CA]: The premises of each food facility must be maintained free of vermin. A facility cannot operate if there is a vermin infestation that leads to contamination of food contact surfaces, packaging, utensils, food equipment, or adulteration of food. The facility is required to cease operations immediately and must remain closed until all corrective actions on the provided checklist are completed. [COS] Live cockroaches were abated and area sanitized.

Observed gap below the screen door in the back of the facility. [CA] Eliminate gap greater than 1/4" to prevent the entrance and harborage of vermin.
