    Employee observed wiping surfaces with a soiled wiping cloth, rinsed their hands in the two compartment handwash/prep sink at the back kitchen area without using soap, then proceeded to handle whole tomatoes. [CA] Employees shall wash their hands in all of the following scenarios: (1) Immediately before engaging in food preparation, including working with nonprepackaged food, clean equipment and utensils, and unwrapped single-use food containers and utensils. (2) After touching bare human body parts other than clean hands and clean, exposed portions of arms. (3) After using the toilet room. (4) After caring for or handling any animal allowed in a food facility pursuant to this part. (5) After coughing, sneezing, using a handkerchief or disposable tissue, using tobacco, eating, or drinking. (6) After handling soiled equipment or utensils. (7) During food preparation, as often as necessary to remove soil and contamination and to prevent cross-contamination when changing tasks. Mop sink observed to have standing water and old food debris in the basin. 10+ flies observed in the unplugged two-door upright cooler at the cook-line. Flies observed crawling on moldy food stored inside the unit. Flies appear to be nesting inside the unit. Per the PIC, facility receives a regular pest control service. Pest control service report could not be provided. [CA] Food facility shall be kept free of non-disease carrying insects, weevils, ants, gnats, and fruit flies. Work with pest control to use any and all approved methods to abate fly presence.

    Sour cream measured 49F, shredded cheese measured 49F, and salsa measured 45F while maintained in improper ice baths at the front service counter. Per staff, items had been taken out of refrigeration less than 1 hour prior and placed into ice baths. [CA] Potentially hazardous foods shall be held at 41F or below, or at 135F or above to prevent the growth of bacteria. [COS] Employee added ice.

    Chicken stored above beef and bacon in the one-door upright cooler at the back kitchen area. Standing pool of meat juice observed at the bottom of the unit. No direct cross contamination observed at time of inspection. [CA] All food shall be separated and protected from cross-contamination. Store all raw meat or other raw products of animal origin below ready-to-eat food items. Ensure all foods are properly packaged/covered/kept in containers to prevent possible contamination.

A follow-up inspection was conducted to verify vermin/pest activity has been abated. K23: No cockroaches observed in the facility. Continue to work on violations noted on the last inspection report. Discussed with operator the urgent need to repair the floors throughout the kitchen area. Cockroaches will drink water in areas where water collect at missing or broken floor tiles.

FOLLOW-UP INSPECTION

On-site for a follow-up inspection after the facility was closed due to evidence of a cockroach infestation on 9/9/2025. Prior to the follow-up inspection, facility provided the following: 1) Pest control report from a licensed provider stating that the facility had been treated, and that no further cockroach presence was observed 2) Completed and signed vermin reopening checklist Facility shall remain closed due to further evidence of a cockroach infestation following treatment. Facility is to immediately cease and desist all operations, including but not limited to food preparation, service, handling, distribution, and sale The following must be provided AGAIN prior to reopening: 1. A new copy of the signed and completed Reopening Checklist, emailed to the assigned inspector 2. A new copy of a new pest control report from a licensed provider, stating that facility has been treated and that all vermin activity has been abated, emailed to the assigned inspector Facility must also perform the following actions prior to reopening: - Eliminate all presence of vermin, dead or alive, within the facility. - Clean and sanitize all areas, including but not limited to floors, walls, ceilings, and surfaces of all equipment throughout the facility - Seal any gaps, cracks, or holes within the premises and in the structure of the facility that may provide harborage or entrance for vermin - Dispose of any contaminated items - Remove unused equipment so as to prevent harborage for vermin A follow-up inspection must be conducted prior to facility reopening. Follow-up for major violations - No evidence of cockroaches found, recommend facility to continue or increase pest control treatment, and place sticky traps throughout facility to ensure facility is free of cockroaches - Reach in cooler next to ice machine does not have food, per PIC, cooler will not be used until it is repaired or replaced - Handwash sink was stocked with soap and paper towels Per Even, employees are told to clean constantly and employees state that those are not part of their job duties. He also states that employees are threatening to sue him. Information of Office of Labor Standards Enforcement was provided.

Failure to comply may result in enforcement action. 2. Facility has (3x) BBQ grills within the dinning room area that may not be in use. Per PIC, they were possibly told by the City not to BBQ outside the facility. [CA] Open-air barbecues or outdoor wood-burning ovens must be operated on the same premises as, in reasonable proximity to, and in conjunction with a permanent food facility that is approved for food preparation and is not operated in any area or location that may constitute a fire hazard. Remove equipment from the facility if BBQ is not allowed.
