    Exterior surfaces of equipment observed with accumulation of grease. Observed food debris on counter tops and shelves. [CA] Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris.

    1. Observed raw beef stored above raw broccoli; no contamination of food observed. [CA] All food shall be separated and protected from cross-contamination. Store all raw meat or other raw products of animal origin below ready-to-eat food items. 2. Observed several food containers missing a lid inside the refrigeration units. [CA] Store open foods in approved NSF containers with tight fitting lids.

    Observed 1 nymph cockroach inside the non operational small ice machine located in the kitchen. PIC removed nymph from facility. No other cockroach activity observed. [CA] No live vermin (cockroach) shall be inside food facility. Clean and sanitize all impacted areas. Specialist will follow up in 1 day to ensure no vermin activity exists in food facility. Consult with professional pest control on how to abate for and monitor for vermin. Maintain premises clean and uncluttered to prevent vermin attraction. Seal all gaps and crevices to prevent vermin harborage.

    Observed chemical bottle stored next to a food product inside the kitchen. [CA] Store chemicals separate and away from the storage and preparation of food or food contact surfaces and linens.

    Observed employee actively ware washing with chlorine and soap in the same middle basin. [CA] Manual warewashing shall be accomplished by using a three-compartment sink where the utensils are first precleaned, washed with hot soapy water, rinsed with hot water, sanitized with an approved method, and air dried. Use 1 TBS of bleach (5.25%) per 1 gal. of water for chlorine solution (100 PPM). Use 1 tbs of quaternary ammonium (10%) per 2 gal. of water for quaternary ammonium (200 PPM). [COS] Specialist explained in Spanish to employee how to properly ware wash and observed ware washing sink properly set up during inspection.
