Facility lacks test strips to measure sanitizer concentration. [CA] Testing equipment and materials shall be provided to adequately measure the applicable sanitization method used during manual or mechanical warewashing.

Heavy build up of duty and grime observed on equipment and equipment surfaces. [CA] Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris.

The only hand wash sink in the kitchen was partially obstructed with empty pitchers maintained inside the wash basin. [CA] Handwashing facility shall be clean, unobstructed, and accessible at all times. [COS] Employee removed obstructions.

Employee's half eaten food observed stored in areas of food preparation. [CA] No employee shall eat or drink in the food prep, food contact storage, or food storage areas, use designated area.

Employee's personal belongings observed stored in areas of food storage. [CA] No person shall store clothing or personal effects in any area used for the storage and preparation of food or food contact surfaces. Employee's personal effects (cell phone, car keys, and jackets) shall be stored in employee lockers or other designated areas for employee storage.

Heavy accumulation of grime, oil, dust, and food residues observed in the following areas: - On wall at the corner of the food preparation area where the induction burners are used to boil water. - On wall behind where all the syrups and sugars are maintained. - On wall behind the hot water dispenser next to the ice machine. - On floors throughout the facility, especially underneath equipment (ice machine, preparation tables, etc.). [CA] Walls and/or floors in food preparation area shall be kept clean. Regularly clean under equipment to prevent accumulation of debris.

Observed container of cooked ground pork maintained out in ambient temperature measured at 48F. Manager stated that an employee had just prepared an order and continued onto other task without returning the PHF item back into the refrigerator. Observed opened gallon container of milk maintained in inadequate ice bath measured at 49F. [CA] PHFs shall be held at 41°F or below or at 135°F or above. [COS] Manager returned items into refrigeration units to immediately cool.

Current health permit not posted in visible location. [CA] The health permit issued by this department shall be posted in a conspicuous place in the food facility.
- K14: Food contact surfaces clean, sanitized
Sanitizer concentration at the three-compartment sink measured less than 50 PPM. Employee conducting manual ware washing observed washing dishes correctly, however in the wrong order. Employee also did not allow sufficient contact time of utensils in solution. [CA] Manual warewashing shall be accomplished by using a three-compartment sink where the utensils are first precleaned, washed with hot soapy water, rinsed with hot water, sanitized with an approved method, and air dried. Use 1 TBS of bleach (5.25%) per 1 gal. of water for chlorine solution (100 PPM). Use 1 tbs of quaternary ammonium (10%) per 2 gal. of water for quaternary ammonium (200 PPM). [COS] Instructed employee to increase concentration of sanitizer as well as re-sanitizing utensils. Observed utensils recently removed from sanitizer stacked in a way that does not allow for proper air drying. [CA] Equipment and utensils must be air dried or properly stored to facilitate drying after sanitizing.

Employees' three-year food handler cards unavailable for review. [CA] Each food handler shall maintain a valid food handler card for the duration of his or her employment as food handler. A valid food handler card shall be provided within 30 days of after the date of hire.
